目的分析眼挫伤继发性青光眼的危险因素.方法回顾性病例研究.收集116例(116眼)眼挫伤继发性青光眼,根据临床表现分为4种类型:房角挫伤型(38例)、眼内积血型(42例)、晶状体异位型(24例)、炎症粘连型(12例).分析其眼压控制及视力恢复情况.结果随访3～6月,116例中有90例(77.6％)视力不同程度提高；97例(83.6％)眼压得到控制,最终恢复正常.不同类型的眼挫伤继发性青光眼临床预后不同.结论房角挫伤和眼内出血是眼挫伤继发性青光眼发生的主要原因,眼球钝挫伤后应密切监测眼压,及时发现继发性青光眼,根据病因及时治疗,避免视功能的损害.

Objective To analyze risk factors associated with secondary glaucoma caused by ocular contusion.Methods The records of 116 patients (116 eyes) were reviewed retrospectively and analyzed.Four different types of secondary glaucoma caused by ocular contusion were classified by clinical findings:chamber angle injury type (38 cases),hemorrhage type (42 cases),lens related (24 cases),and proliferation type (12 cases).The intraocular pressare and the visual acuity outcome were analyzed.Results After follow-up for 3-6 months,the intraocular pressure (IOP) was successfully controlled without anti-glaucoma medical treatment in 97 eyes (83.6％).The visual acuity was improved in 90 cases (77.6％).Different type of secondary glaucoma caused by ocular contusion showed very different prognosis.Conclusion The main reasons of secondary glaucoma caused by ocular contusion were chamber angle injury and hemorrhage.IOP should be examined after ocular contusion.Then secondary glaucoma could be discovered in time.Each subtype of patients should take relevant treatments,so as to avoid the further damages to the visual function.
