目的 探讨经巩膜固定的人工晶状体(IOL)半脱位的原因和治疗方法.方法 回顾性病例系列.研究2004年1月至2012年6月在北京同仁眼科中心连续治疗的经巩膜固定的IOL半脱位41例(43只眼).均先行眼底检查,对于不适于再保留IOL者,摘出原IOL;可以继续植入IOL者,根据眼部和IOL状况,分别采用选取新的巩膜位置进行缝合,固定脱位IOL襻,或换用新的IOL缝线固定.术后随访4 ～ 98个月.结果 43只眼均顺利完成手术.2只眼摘出IOL后,未再植入；20只眼摘出原IOL,采用新IOL再次植入.14眼直接固定脱位IOL襻.7眼摘出原IOL后,仍用该IOL再次植入.最后随访时最佳矫正视力＞0.3者20只眼.眼压11～23 mm Hg(1 mm Hg=0.133 kPa).结论 IOL脱位襻固定或IOL重新缝线固定是治疗经巩膜固定的IOL半脱位的有效方法,可以获得较好的视功能,但应注意长期随访,预防术后并发症.
目的 探討經鞏膜固定的人工晶狀體(IOL)半脫位的原因和治療方法.方法 迴顧性病例繫列.研究2004年1月至2012年6月在北京同仁眼科中心連續治療的經鞏膜固定的IOL半脫位41例(43隻眼).均先行眼底檢查,對于不適于再保留IOL者,摘齣原IOL;可以繼續植入IOL者,根據眼部和IOL狀況,分彆採用選取新的鞏膜位置進行縫閤,固定脫位IOL襻,或換用新的IOL縫線固定.術後隨訪4 ～ 98箇月.結果 43隻眼均順利完成手術.2隻眼摘齣IOL後,未再植入；20隻眼摘齣原IOL,採用新IOL再次植入.14眼直接固定脫位IOL襻.7眼摘齣原IOL後,仍用該IOL再次植入.最後隨訪時最佳矯正視力＞0.3者20隻眼.眼壓11～23 mm Hg(1 mm Hg=0.133 kPa).結論 IOL脫位襻固定或IOL重新縫線固定是治療經鞏膜固定的IOL半脫位的有效方法,可以穫得較好的視功能,但應註意長期隨訪,預防術後併髮癥.

Objective To analyze the Causes and the therapies of the subluxation of transscleral sutured posterior chamber intraocular lens.Methods Retrospective cases series.A retrospective analysis of 43 eyes of 41 patients were reviewed who underwent medical treatment of the subluxation of posterior chamber lenses with transscleral sutures from January 2004 to June 2012.All 43 eyes were given priority to fundus examination.According to the condition,procedures for repairing were performed including the previously implanted IOL removed,the dislocated haptic refixated and the IOL resutured on a new position of the sclera,or new IOL implanted.The duration of follow-up was 4 to 98 months.Results The surgical procedures were performed safely and successfully in all patients.Two IOLs were removed forever,while the subluxation of IOLs was replaced by new ones in 20 eyes respectively.The dislocated haptics were refixated in 14 eyes,and the subluxaed IOLs were resutured in 7 eyes.The best visual acuity was ＞0.3 in 20 eyes(46.5％) at the last follow-up after surgery.The range of IOP was 11-23mmHg(1 mmHg =0.133 kPa).Conclusion It is an effective method to refixate the dislocated haptic and resutured the IOL for treatment of subluxation of transscleral sutured posterior chamber IOL.Moreover,it will prevent the postoperative complications in patients by paying more attention to long-term follow-up.
