目的探讨双切口青光眼白内障联合手术的手术源性散光及对角膜内皮细胞的影响.方法 28例(34只眼)慢性闭角型青光眼合并白内障行双切口青光眼白内障联合手术,以角膜曲率仪测量术前及术后1周、1个月、3个月的角膜散光度及角膜中央内皮细胞密度,采用矢量分析法计算手术源性散光值.结果术后1周平均角膜散光度与术前相比,差异有统计学意(P＜0.05).术后1个月、3个月与术前相比,差异无统计学意义(P＞0.05).术后1个月、3个月手术源性散光值与术后l周相比,差异有统计学意义(P＜0.05),术后3个月手术源性散光值与术后1个月相比,差异无统计学意义(P＞0.05).术后1周、1个月、3个月角膜内皮细胞密度与术前相比,差异有统计学意(P＜0.05),术后1个月、3个月角膜内皮细胞密度与术后1周相比,差异无统计学意义(P＞0.05).术后1个月、3个月角膜内皮细胞丢失率与术后1周相比,差异无统计学意义(P＞0.05).结论双切口青白联合手术所致手术源性散光值低,对角膜内皮细胞损伤小.

Objective To evaluate the clinical effects of dual-incision surgery of phacoemulsification and intraocular lens implantation combined with trabeculectomy on surgically induced astigmatism(SIA) and corneal endothelialium.Methods Thirty-four eyes of 28 patients with glaucoma and cataract were enrolled in the study and treated by dual-incision phacoemulsification and intraocular lens implantation combined with trabeculectomy.The corneal astigmatism and density of central corneal endothelia were measured preoperatively,1 week,1 month and 3 months postoperatively.SIA was calculated using Vector Analysis method.Corneal endothelial density and loss rate were calculated by specular microscopy.Results Astigmatism was significantly different between the values before surgery and 1 week after surgery (P ＜ 0.05).There were no significant differences among the results of astigmatism before surgery and 1 months,3 months after surgery (P ＞ 0.05).SIA were significantly different among the results at 1 week and 1 month,3 months after surgery (P ＜ 0.05).There was no significant difference in SIA at 1 month and 3 months after surgery (P ＞ 0.05).There was statistically significant difference in corneal endothelial cell density among the values before surgery and 1week,1 month,3 months after surgery (P ＜ 0.05).There was no statistically significant difference in corneal endothelial cell density among the results at 1 week and 1 month,3 months after surgery (P ＞ 0.05).There was no significant difference in corneal endothelial loss rate among the values at 1 week,1 months,3 months after surgery (P ＞ 0.05).Conclusion The dual-incision phacoemul-sification combined with trabeculectomy surgery cause low rates of endothelial cell loss and SIA.
