目的探讨外伤性泪小管断裂显微吻合并双路置管手术的效果.方法 79例(79眼)均在手术显微镜直视下或黏弹剂帮助下寻找到泪小管鼻侧断端,以双路硅胶导管为支撑物,置管2～3个月,拔管后随访3 ～ 24个月.结果结果77例(77眼)随访,65眼(84.42％)泪道冲洗通畅,无溢泪;8眼(10.39％)泪道冲洗通畅,有溢泪;4眼(5.19％)通而不畅.主要并发症为瘢痕增生导致失败.结论手术显微镜下泪小管断裂修复并双路置管手术治疗外伤性泪小管断裂效果良好.
목적 탐토외상성루소관단렬현미문합병쌍로치관수술적효과.방법 79례(79안)균재수술현미경직시하혹점탄제방조하심조도루소관비측단단,이쌍로규효도관위지탱물,치관2～3개월,발관후수방3 ～ 24개월.결과 결과77례(77안)수방,65안(84.42％)루도충세통창,무일루;8안(10.39％)루도충세통창,유일루;4안(5.19％)통이불창.주요병발증위반흔증생도치실패.결론 수술현미경하루소관단렬수복병쌍로치관수술치료외상성루소관단렬효과량호.
Objective To observe the clinical efficacy of double silicone intubation for repair of traumatic canalicular laceration.Methods 79 cases (79 eyes) of traumatic canalicular laceration were anastomized under microscope.The nasal broken ending was found with the help of viscoelastic agent.The bicanalicular silicon tubes were inserted and left in the lacrimal passage for 2 ～ 3 mnonths.The patients were followed-up for 3 ～24 months after the tubes were removed.Results 77 cases (77 eyes) were followed up.65 eyes (84.42％) had patent lacrimal passages and no epiphora.8 eyes (10.39％) had epiphora with patent lacrimal passages.4 eyes (5.19％) had obstruction in lacrimal passages.The main complication was scar formation which caused surgery to fail.Conclusion Double silicone intubation under microscope has good clinical efficacy for repair of traumatic lacrimal canalicular laceration.
