目的探讨改良下睑缘皮肤切口联合口内前庭沟切口入路手术治疗颧弓复合体骨折的效果.方法对颧弓复合体骨折整复手术49例进行回顾性研究,分析其致伤原因、临床表现、眼球突出度、复视像和同视机检查结果.49例全部采用改良下睑缘皮肤切口联合口内前庭沟切口入路修复手术.结果术后14 d,全部患者眶周软组织肿胀消退,颧部畸形得到完全矫正,张口度均较术前明显好转.术后3个月,所有患者双侧颧部形态对称,外观无差异,张口度恢复超过40 mm.双眼球突出度对称;1例复视明显者需作斜视手术调整眼肌.结论颧弓复合体骨折手术采用改良下睑缘切口联合口内前庭沟切口入路,绝大部分患者可以得到良好的治疗效果.

Objective To evaluate the efficacy of surgerical treatment of zygomatic complex fracture by improved subciliary skin incision combined with oral vestibular incision.Methods A retrospective research was carried out with 49 zygomatic complex fracture patients.Many factors were observed,including injury causes,clinical manifestations and accompanying symptoms,degree of exophthalmos,diplopia and synoptophore examinations.Results After 14 days of operation,periorbital soft tissue swelling of all patients was alleviated,malar deformity was completely corrected,mouth opening degree was significantly improved compared with preoperative.At 3 months after operation,all patients were symmetrical in the bilateral zygomatic complex,with no differences in appearance,mouth opening degree recovered more than 40 mm.The degrees of exophthalmos were the same in the two sides.One patient with apperent diplopia was required for strabismus operation to adjust extraocular muscles.Conclusion Zygomatic complex fracture operation should adopt the improved subciliary skin incision combined with oral vestibular incision,and most patients can obtain satisfactory therapeutic effect.
