目的 探讨改良式小切口非超声乳化白内障摘出人工晶状体植入术在高龄硬核白内障的中的临床应用.方法 对82例高龄患者104眼硬核白内障施行改良式小切口非超声乳化白内障摘出人工晶状体植入术,并与同期81例高龄患者103眼硬核白内障施行晶状体超声乳化吸出人工晶状体植入术对比,术后随访12个月,比较两种手术时间、角膜内皮细胞的密度、丢失率,以及术后视力.结果 两种手术方式的手术时间差异无统计学意义(P>0.05),在角膜内皮细胞密度、丢失率和术后视力的比较差异有统计学意义(P<0.05).结论 高龄硬核白内障适宜行改良式小切口非超声乳化白内障摘出人工晶状体植入术,且有损伤小、恢复快等优点.

목적 탐토개량식소절구비초성유화백내장적출인공정상체식입술재고령경핵백내장적중적림상응용.방법 대82례고령환자104안경핵백내장시행개량식소절구비초성유화백내장적출인공정상체식입술,병여동기81례고령환자103안경핵백내장시행정상체초성유화흡출인공정상체식입술대비,술후수방12개월,비교량충수술시간、각막내피세포적밀도、주실솔,이급술후시력.결과 량충수술방식적수술시간차이무통계학의의(P>0.05),재각막내피세포밀도、주실솔화술후시력적비교차이유통계학의의(P<0.05).결론 고령경핵백내장괄의행개량식소절구비초성유화백내장적출인공정상체식입술,차유손상소、회복쾌등우점.
Objective To investigate the application of improved small incision nonphacoemulsification cataract extraction and intraocular lens implantation in senile cataract with hard nucleus.Methods 207 eyes of 163 cases of senile cataract underwent small incision nonphacoemulsification or phacoemulsification cataract extraction combined with intraocular lens implantation and the patients were followed up for 12 months.Based on the different procedures,the patients were divided into two groups:small incision nonphacoemulsification group (104 eyes of 82 cases) and phacoemulsification group (103 eyes of 81 cases).The time of surgery,the change of density and loss rate in corneal endothelial cells,and the vision of patients of the two groups were compared.Results The difference of operation time of two groups had no statistical significance(P > 0.05).The changes of density and loss rate in corneal endothelial cells and the vision of patients in two groups were statistically significantly different (P < 0.05).Conclusion Small incision nonphacoemulsification and intraocular lens implantation is a safe and effective surgical treatment for hard nucleus cataract.
