目的 观察水下连续环形撕囊技术在手法小切口硬核白内障手术中的临床效果.方法 我院白内障236例(278眼).年龄53 ~88岁.晶状体核硬度Ⅳ~Ⅴ级.随机分为两组:常规撕囊组(122眼):晶状体核硬度Ⅳ级72眼,Ⅴ级50眼,水下撕囊组(156眼):晶状体核硬度Ⅳ级94眼,Ⅴ级62眼.比较两组环形撕囊成功率、手术源性散光度、术中术后并发症.随访3个月.结果 水下撕囊组成功率显著高于常规撕囊组(x2 =8.818,P<0.01).两组手术前后散光度差异无统计学意义,组间术后散光差异无统计学意义.常规撕囊组中有3眼后囊破裂,水下撕囊组中有2眼后囊小范围破裂.结论 水下环形撕囊技术便捷易行、安全实用,适合成熟期和过熟期白内障的各种硬度的核.

Objective To investigate the clinical efficacy of underwater continuous circular capsulorhexis (CCC) technology in small incision hard nucleus cataract operation.Methods 278 eyes of 236 cases of cataract with Ⅳ-Ⅴ hard nuclei were enrolled in the study and patients' ages ranged from 53 to 88 years old.All cases were divided into two groups:conventional CCC group and underwater CCC group.There were 122 eyes(72 eyes with Ⅳ hard nuclei and 50 eyes with Ⅴ hard nuclei)in conventional CCC group,all of which accepted conventional CCC.There were 156 eyes (94 eyes with Ⅳ hard nuclei and 62 eyes with Ⅴhard nuclei) in underwater CCC group,all of which received underwater CCC.The surgical success rate,surgically induced astigmatism,intraoperative and postoperative complications were compared between two groups.The follow-up was 3 months.Results The surgical success rate of underwater CCC group was higher than that of conventional CCC group significantly (x2 =8.818,P < 0.01).The changes of degree of astigmatism before and after surgery had no statistically significant in both two groups.Posterior capsular rupture was found in 3 eyes in conventional CCC group and 2 eyes in underwater CCC group.Conclusion Underwater CCC is safe and easy to operate,which is suitable for mature and hypermature cataract with different degrees of hard nuclei.
