目的探讨原发性视网膜脱离伴玻璃体积血的手术方式及治疗效果.方法对36例(36眼)原发性视网膜脱离伴玻璃体积血的患者行玻璃体切除手术和眼内激光光凝封闭视网膜裂孔,结合环扎加压术或眼内惰性气体C3F8填充.结果术后随访6个月以上,36例(36眼)患者3眼视力无提高,其中1例患者术后再次发生视网膜脱离二次玻切术视网膜复位和硅油填充,2例因白内障加重视力下降,行超声乳化白内障摘除联合人工晶体植入术,术后视力均较术前提高.其余33眼术后视力均较术前提高,视网膜完全复位,裂孔封闭,无玻璃体积血复发.结论原发性视网膜脱离伴玻璃体积血时早期行玻璃体切除和眼内激光光凝封闭视网膜裂孔结合环扎加压术或眼内气体填允是有效的、可靠的方法,且疗效明显.
목적 탐토원발성시망막탈리반파리체적혈적수술방식급치료효과.방법 대36례(36안)원발성시망막탈리반파리체적혈적환자행파리체절제수술화안내격광광응봉폐시망막렬공,결합배찰가압술혹안내타성기체C3F8전충.결과 술후수방6개월이상,36례(36안)환자3안시력무제고,기중1례환자술후재차발생시망막탈리이차파절술시망막복위화규유전충,2례인백내장가중시력하강,행초성유화백내장적제연합인공정체식입술,술후시력균교술전제고.기여33안술후시력균교술전제고,시망막완전복위,렬공봉폐,무파리체적혈복발.결론 원발성시망막탈리반파리체적혈시조기행파리체절제화안내격광광응봉폐시망막렬공결합배찰가압술혹안내기체전윤시유효적、가고적방법,차료효명현.
Objective To evaluate the significance of the surgery for primary retinal detachment accompany with vitreous hemorrhage.Methods A retrospective study was done in 36 patients(36 eyes)with primary retinal detachment accompany with vitreous hemorrhage whose eyes were treated with vitreetomy,endolaser coagulation and association with silicone buckling or C3F8 padding.Results Visual acuity(VA) of the patients was improved after operation in 33 cases.And retinal reattachment was achieved and vitreous hemorrhage did not recur.The VA of two patients was dropped because of the complicated cataract.And the VA of the two patients was recovered after phacoemulsification and intraocular lens implantation.One patient was performed vitrectomy again twice because of recurred retinal detachment.Following up the patients was over six months.Conclusion In the cases of primary retinal detachment accompany with vitreous hemorrhage,pars plena vitrectomy and prompt laser photacoagulation or silicone buckling is effective and dependable method.
