目的 探讨高度近视患者接受波前像差引导LASIK术后2年在暗环境下无眩光、低眩光及高眩光条件下对比敏感度的变化.方法 临床病例对照研究.对2009年9月至2010年2月在西安市第四医院眼科接受波前像差引导LASIK手术的高度近视患者19例38只眼作为波前组,同期行常规LASIK手术患者18例36只眼作为对照组.分别对两组术前及术后2年在暗环境下无眩光、低眩光及高眩光条件下各空间频率的对比敏感度标准化Log值(contrast sensitivity function,CSF)比较(主视眼).结果 术后波前组除在暗环境高眩光环境下12 CPD、18 CPD频段CSF比较无统计学意义(t=1.689,1.645,P>0.05),其余各种环境下CSF均增加(P<0.05),眩光刺激对于波前组术前术后各空间频率的CSF均无影响(P>0.05);对照组除在暗环境下3 CPD频段及无眩光6 CPD频段CSF差异无统计学意义(t =1.711,1.862,1.902,1.867,P>0.05),其余各种环境下CSF与术前相比均明显降低差异有统计学意义(P<0.05);眩光刺激对于对照组术前各空间频率的CSF无影响(P>0.05);暗环境下高眩光刺激降低了对照组术后12、18 CPD频段CSF差异有统计学意义(t=1.432,1.561,P<0.05).结论 高度近视患者,波前像差引导的LASIK手术后2年暗环境下CSF优于常规LASIK对照组,且较术前略有增加;眩光刺激对于波前像差引导的LASIK手术前后各空间频率的CSF无影响,暗环境下高眩光刺激可影响常规LASIK对照组术后中高频段的CSF.

Objective To investigate changes of contrast sensitivity under dark circumstance with or without glare before and after standardized LASIK and wavefront-guided LASIK in high myopic eyes at two years follow up.Methods There were 19 high myopic patients (38 eyes) performed wavefront-guided LASIK as experimental group,the other 18 high myopic patients (36 eyes) performed with standardized LASIK as control group.Contrast sensitivity was detected under photopic and mesopic circumstances.The contrast sensitivity (CS) of patients was measured by CSV-1000E 2 years after the procedure.Changes of contrast sensitivity were compared.Results When tested at dark circumstance with glare background,the CS of 12 and 18 c/d had no significant difference,however,the CS increased under other circumstance after wavefront group.Glare did not have an influence on contrast sensitivity before and after wavefront-guided LASIK group.In control group,the CS of 3 c/d and 6 c/d under dark circumstance without glare background had no significant difference before and after the procedure.While the CS decreased under other circumstance in this group.There was an influence on 12 and 18 c/d under dark circumstance with glare background after standard group.Except there was no difference under dark circumstance without glare background,the contrast sensitivity in the wavefront-guided LASIK group were better than the standard LASIK.There were 38 patients expressing satisfy in wavefront group and 36 patients in control group through the visual quality questionnaire,significant difference was found.Conclusions The contrast sensitivity in the wavefront-guided LASIK group is better than the standard LASIK.Glare do not have an influence on contrast sensitivity before and after wavefront-guided LASIK group,while there is an influence on mesopic and high spatial frequency contrast sensitivity function in dark circumstance after standard group.
