目的比较Byron Smith和外侧睑板条固定术治疗老年性退行性下睑外翻的手术效果.方法 2008年1月至2012年12月对38例(52只眼)老年性中重度退行性下睑外翻患者随机分为2组,1组20例(28只眼)行Byron Smith术;2组18例(24只眼)行外侧睑板条固定术.术后随访观察6个月至1年,观察比较矫正效果及并发症情况.结果 38例52只眼下睑外翻全部矫正,睫毛生长方向正常,结膜充血、肥厚、溢泪症状消失.1组术后睑缘紧密贴附眼球者25只眼,睑球结膜炎症消退,轻度睑球分离者3只眼;睑缘平滑26只眼,睑缘切迹2只眼.2组下睑缘紧贴眼球者20只眼,均无睑球分离,睑缘平滑;其中10只眼早期睑裂变小,术后6个月睑裂大小恢复.结论 Byron Smith术和外侧睑板条固定术均可有效矫正退行性下睑外翻,其中Byron Smith术疗效可靠,操作简便,对轻中度睑外翻有明显的矫正效果,外侧睑板条固定术操作相对复杂,但矫正力度较大,对中重度下睑外翻有更确切地矫正效果.

Objective To compare the two surgical procedures for involutional ectropion correction of lower eyelid and evaluate their efficacy.Methods From January 2008 to June 2010,one surgeon performed lower-lid correction operation using the Byron Smith method for 24 patients (a total of 32 eyes).Afterwards,the same surgeon performed the lateral tarsal strip fixation method for 13 patients (a total of 20 eyes) during March 2011 to December 2012.Patients were followed up for 6 months to 1 year,and the effect and complication were monitored and compared.Results The ectropion in all patients (52 eyes total) was rectified and ciliary orientation was acceptable at the end of the surgery,and there was no recurrence and epiphora in any patients of two groups after at least 6 months follow-up.The cases in Byron Smith method group reported two complications:mild eyelid separation (3 eyes) and minimal eyelid margin notching (2 eyes).However,the cases in the lateral tarsal strip fixation procedure complained tension and blepharophimosis (10 eyes including 3 recurrences) in early period after operation,and got right after 6 months follow-up.Conclusions The Byron Smith method and Lateral tarsal strip fixation method both can correct involutional ectropion of low eyelids.However,Byron Smith method is easy and simple to handle mild to moderate lower eyelid ectropion with curative effect,but for severe lower eyelid ectropion,the lateral tarsal strip fixation method is more appropriate,though it is complicated comparatively.
