目的 探讨CD4+ CD25+ Treg在视网膜母细胞瘤(retinoblastoma,RB)发生免疫逃逸中的作用.方法 收集2007年1~12月在中山眼科中心诊治的RB患者31例.临床资料,设立实验组与对照组.将RB患者作为实验组,共31例;阴性对照组:Coats病患儿1例及眼球破裂伤患儿7例.以流式细胞仪检测患者外周血及组织局部CD4+ CD25+ Treg的比例.又以同样方法检测了7例眼睑恶性肿瘤、11例泪腺恶性肿瘤、13例泪腺良性肿瘤及8例视神经良性肿瘤患者肿瘤局部及外周血中CD4+CD25+ Treg的比例.结果 RB患者肿瘤局部较阴性对照组组织局部CD4+ CD25+ Treg的比例明显增高,分别为(21.56±6.83)%、(9.33±4.45)%,2个组差异具有统计学意义(t=4.51,P=0.032);2个组患者外周血中CD4+ CD25+ Treg的比例分别为(9.51±3.98)%、(8.48±3.43)%,差异无统计学意义(t =0.079,P=0.217).眼眶恶性肿瘤(眼睑、泪腺恶性肿瘤)局部CD4+ CD25+ Treg比例均较高,分别为(23.62±8.62)%、(29.74±9.22)%,而眼眶良性肿瘤(泪腺、视神经良性肿瘤)局部CD4+ CD25+ Treg比例则较低,分别为(12.61±5.96)%、(8.03±3.57)%,无论眼眶恶性肿瘤还是良性肿瘤患者外周血中CD4+ CD25+ Treg比例较正常人群均无明显增高.结论 RB患者肿瘤局部CD4+CD25+ Treg比例增高可能是引起视网膜母细胞瘤肿瘤局部发生免疫逃逸的原因之一.

Objective To investigate the effect of CD4+CD25+ Regulatory T cell in the tumor immune escape mechanism in patients with retinoblastoma.Methods Flow cytometric analysis was used to quantify the prevalence of CD4+CD25+ Regulatory T cell in the peripheral blood and fresh tissue samples from the patients with retinoblastoma (31 eyes).Peripheral blood samples and retina tissue were also collected from the patients with coats disease (one eye) and the patients with eyeball rupture (7 eyes) for comparison.Moreover,the same method was used to quantify the prevalence of CD4+CD25+ Regulatory T cell in the peripheral blood and fresh tissue samples from the patients with eyelid malignant tumor (7 eyes),malignant tumor of lacrimal gland (11 eyes),benign tumor of lacrimal gland (13 eyes),and benign tumor of the optic nerve (8 eyes).Results The percentages of CD4+CD25+ Regulatory T cell in the tumor sample of the patients with retinoblastoma were significantly higher than that of control group (21.56±6.83% and 9.33±4.45% respectively,t =4.51,P =0.032).Compared with control group,the proportion of CD4+CD25+ Regulatory T cell in peripheral blood of patients with retinoblastoma,there was no significant difference (9.51±3.98% and 8.48±3.43% respectively,t =0.019,P =0.217).The percentages of CD4+CD25+ Regulatory T cell in the tumor sample of the patients with eyelid malignant tumor and malignant tumor of lacrimal gland was higher (23.62±8.62% and 29.74±9.22% respectively),and which in the patients with lacrimal gland benign tumor and the optic nerve benign tumor was lower (12.61±5.96% and 8.03±3.57%,respectively).Compared with the normal people,there was no significant increased in the proportion of CD4+CD25+ Regulatory T cell in peripheral blood of patients with malignant tumor or benign tumor of orbit.Conclusions This research show that the expression of CD4+CD25+ Regulatory T cell is increased in the local tumor micro-environment of the patients with retinoblastomal.That may be one factor of the mechanism of tumor immune escape.
