目的 探讨Lenstar LS900、IOL Master、Orbscan-Ⅱ三种不同仪器测角膜直径和前房深度有无差异.方法 对2013年2~3月在武警河南总队医院屈光手术中心就诊的近视患者63例(126只眼),分别采用Lenstar LS900、IOL Master、Orbscan-Ⅱ测量角膜直径和前房深度,三种仪器均测量3次及以上,取其平均值,并对所得数据进行统计学分析.结果 (1)差异性:三种仪器测量角膜直径值分别为(11.95±0.38) mm、(11.84±0.33) mm、(11.52±0.31) mm,差异均有统计学意义(F=193.15,P=0.004),用SNK-q多个均数间全面比较差异均有统计学意义(Pall≤0.05);测得前房深度值分别为(3.68±0.21) mm、(3.67±0.21) mm、(3.60±0.20) mm,差异均有统计学意义(F =3351.40,P =0.001),用SNK-q多个均数间全面比较,Lenstar LS900与IOL Master测量差异均无统计学意义(q=64.21,P=0.135),Orbscan-Ⅱ测量与Lenstar LS900、IOL Master相比差异均有统计学意义(Pall≤0.05).(2)相关性:角膜直径测量中,相关分析显示三种仪器的测量结果均有显著相关关系,相关系数分别为0.795(P=0.008)、0.754 (P =0.014)、0.637(P=0.019);前房深度测量中,相关分析显示三种仪器测量均有显著相关关系,相关系数分别为0.923 (P =0.010)、0.895 (P =0.013)、0.905 (P =0.023).(3)一致性:Bland-Ahman分析显示三种设备测角膜直径和前房深度一致性较好.结论 角膜直径和前房深度测量,Lenstar LS900与IOL Master测量值相近,相对Orbscan-Ⅱ较大;三种设备之间均有较高的正相关性;三种设备有较好的一致性,其中Orbscan-Ⅱ重复性最好.

Objective To compare the differences of corneal diameter and anterior chamber depth measured by the Lenstar LS900,IOL Master and Orbscan-Ⅱ.Methods Sixty-three patients with myopia (126 eyes) were involved,and they were the ones who came to Henan Crops Hospital of Armed Police Force in February to March in 2013.Sequential measurements of corneal diameter and anterior chamber depth were performed by the same operator with Lenstar LS900,IOL Master and Orbscan-Ⅱ.Three instruments were measured for 3 times or more.The average value and the data were obtained and statistically analyzed.Results 1.difference:The averages of WTW measured by three instruments were (11.95±0.38)mm,(11.84±0.33)mm and (11.52±0.31)mm.The difference of corneal diameter measured by three different instruments was statistically significant (F =193.15,P =0.004),and the differences between each two instruments were statistically significant,too (Pall≤ 0.05); The averages of ACD measured by Lenstar LS900,IOL Master and Orbscan-Ⅱ were (3.68± 0.21)mm,(3.67±0.21)mm and (3.60±0.20)mm.The difference of anterior chamber depth measured by three consistence instruments was statistically significant (F =3351.40,P =0.001).The difference between Lenstar LS900 and IOL Master was not statistically significant (q=64.21,P =0.135),but there were statistical significance in difference between Orbscan-Ⅱ and Lenstar LS900 (Pall≤0.05).The comparison between IOL Master and Orbscan-Ⅱ was the same result.2.correlation:Positive correlationship existed each two WTW results measured by Lenstar LS900,IOL Master and Orbscan-ii,and the correlation coefficients were 0.795 (P =0.008),0.754 (P =0.014),0.637 (P =0.019); There were also positive correlationship between ACD results measured by Lenstar LS900,IOL Master and Orbscan-Ⅱ,and the correlation coefficients were 0.923 (P =0.010),0.895 (P =0.013),0.905 (P =0.023).3.consistence:Bland-Altman analysis showed that 95% consistency limit of WTW and ACD measured by three different instruments were both good.Conclusions The results of Lenstar LS900 and IOL Master are the most similar in the measurement of WTW and ACD,and they are larger than that of Orbscan-Ⅱ.The three instruments have high positive correlation and consistency.The instrument which has the best repeatability is Orbscan-Ⅱ.
