目的观察调节对近视发展的影响.方法根据年龄、屈光状态,按就诊顺序随机选取2012年3月至2013年3月在沈阳爱尔眼视光医院门诊验光配镜的在校近视眼学生37例(74只眼),其中女22例,男15例,年龄9～12岁,平均(11.08±1.66)岁.屈光状态:球镜(-2.59±1.27)D,柱镜(-0.63±0.91)D.综合验光仪检查正、负相对调节(PRA/NRA)、调节反应(BCC)后予复方托吡卡胺滴眼液快速散瞳验光配镜.一年后由同一验光师再次做以上检查.比较一年后近视的发展与PRA、NRA、BCC的关系.结果一年后近视增长(-0.74±0.47)D,PRA与一年近视增长呈负相关(r =-0.297,P=0.01);NRA、BCC与一年近视增长无相关,均P＞0.05.结论 PRA与近视发展呈负相关,即调节力越强近视发展越慢,NRA、BCC与近视发展无关.

Objective To investigate the progression of myopia with different positive,negative accommodation and accommodative facility.Methods Thirty-seven students were randomly chose for the study.Refraction,positive,negative accommodation and accommodative facility tests were performed on the students.After 12 months,repeated again.Results were analyzed with SPSS software.Results The mean progression of refraction in schoolchildren was significant (P =0.00).The progression of myopia was significantly related to positive accommodation (P =0.01),and it was not related to negative accommodation and accommodative facility (P ＞0.05).Conclusions The progression of myopia is significantly related to accommodative lag while reading.
