目的 通过检测P-gp、GST-π和LRP在眼眶淋巴瘤组织芯片中的表达情况,从耐药角度探索该病的复发及预后不良机制.方法 收集天津市眼科医院及天津市第一中心医院眼科于2006年9月至2011年6月经手术切除且病理诊断明确的50例(其中46例为观察组)眼眶淋巴瘤石蜡标本,另取眼眶反应性淋巴增生17例作为对照组.制作眼眶淋巴瘤组织芯片,应用免疫组织化学技术检测P-gp、GST-π、LRP在眼眶淋巴瘤中的表达.并对46例眼眶淋巴瘤病人预后进行随访,以比较不同预后患者淋巴瘤组织中P-gp、GST-π、LRP的表达情况.结果 P-gp在眼眶淋巴瘤和反应性淋巴增生组织芯片中的阳性表达率分别为87.0% (40/46)和35.3% (6/17),其差异有统计学意义(x2=16.81,P=0.000):GST-π在以上两种病变的阳性表达率分别为76.1% (35/46)和41.2% (7/17),其差异有统计学意义(x2=6.808,P=0.009); LRP在两种组织中均为阴性表达.在眼眶淋巴瘤预后不良和预后良好之间,P-gp的阳性表达率分别为85.7% (12/14)和42.9% (9/21),经Fisher检验,P=0.033,其差异有统计学意义;GST-π在以上两种情况间的阳性表达率分别为78.6% (11/14)和38.1% (8/21),经Fisher检验,P=0.013,其差异有统计学意义.结论 眼眶淋巴瘤组织芯片省时快速、高效经济地研究了该肿瘤的多药耐药基因表达状况.眼眶淋巴瘤的多药耐药性与P-gp和GST-π有关.

Objective To explore the recurrence and poor prognosis mechanism from the perspective multi-drug resistance (MDR) by detecting the expression of P-gp,GST-π and LRP in the orbital lymphoma.Methods Fifty orbital lymphoma tissue which were diagnosed by pathology after surgery were collected from Tianjin eye hospital and Tianjin first centre hospital.Among them,46 cases were the observation group.Meanwhile,17 reactive lymphoid hyperplasia tissues were collected as contrast group.Orbital lymphoma tissue microarray and immunohistochemical staining was used to detect the expression of P-gp,GST-π and LRP in orbital lymphoma and reactive lymphoid hyperplasia.Results The P-gp expression rates were 87.0%(40/46) and 35.3%(6/17) in the two tumors tissue microarray and there was significant difference between them (x2=16.81,P =0.000).The GST-π expression rates were 76.1%(35/46) and 41.2%(7/17) in the two tumors tissue microarray and there was significant difference between them (x2=6.808,P =0.009).In poor prognosis group and good prognosis group of the orbital lymphoma,the P-gp expression rates were 85.7%(12/14) and 42.9%(9/21) and there was significant difference between them (Fisher test,P =0.033).The GST-π expression rates were 78.6%(11/14) and 38.1% (8/21) in the two groups and there was significant difference between them (Fisher test,P=0.013).Conclusions By orbital lymphoma tissue microarray,the MDR gene expression can be detected rapidly,it is efficiently and economically.The MDR of orbital lymphoma is associated with the big expression of P-gp and GST-π.
