目的 探讨肝功能正常的HBV感染者肝组织HBV共价闭合环状DNA(HBV cccDNA)与血清HBV DNA、肝组织总HBV DNA之间的相关性.方法 实时荧光定量PCR检测125例肝功能正常的HBV感染者的血清、肝组织总HBV DNA和肝组织HBV cccDNA定量.正态分布数据采用直线相关分析,非正态分布数据采用非参数统计.结果 HBV感染者血清HBVDNA为(5.87±1.47) lg IU/mL,肝组织总HBV DNA为(6.63±1.27) lg拷贝/106细胞;HBV cccDNA为(5.66±0.95) lg拷贝/106细胞.血清HBV DNA与肝组织总HBV DNA呈正相关(r=0.743,P<0.01),血清HBV DNA与肝组织HBV cccDNA呈正相关(r=0.417,P<0.01),肝组织总HBV DNA与肝组织HBV cccDNA呈正相关(r=0.443,P<0.01).肝组织HBV cccDNA在31~40岁年龄段最高,血清HBV DNA和肝组织总HBV DNA则在20~30岁年龄段最高.64例(51.2%)HBV感染者肝组织病理学提示为<G2S2,HBV cccDNA水平与肝组织炎症活动度分级和纤维化分期无关(x2 =0.048,P>0.05).结论 肝功能正常的HBV感染者肝组织HBV cccDNA与肝内总HBV DNA、血清HBV DNA呈正相关,不同年龄HBV感染者血清和肝组织内HBV复制水平存在差异,HBV cccDNA水平与肝组织病理学无关.

Objective To investigate the correlation between the intrahepatic hepatitis B virus (HBV) covalently closed circular DNA (cccDNA),serum HBV DNA,and intrahepatic total HBV DNA in HBV-infected patients with normal liver function.Methods Liver biopsy specimens and serum samples were obtained from 125 asymptomatic HBV-infected patients with normal liver function.Serum HBV DNA,intrahepatic total HBV DNA and HBV cccDNA were quantitatively determined by real time fluorescence polymerase chain reaction (PCR).Linear correlation analysis was used to determine the relationships between normally distributed data.For data with abnormal distributions,nonparametric analysis was used.Results In HBV-infected patients with normal liver function,serum HBV DNA level was (5.87±1.47) lg IU/mL,intrahepatic total HBV DNA level was (6.63±1.27) copy/106 cells and intrahepatic HBV cccDNA was (5.66±0.95) copy/106 cells.Serum HBV DNA was correlated positively with both the total intrahepatic HBV DNA (r=0.743,P<0.01)and with intrahepatic HBV cccDNA (r=0.417,P<0.01).The total intrahepatic HBV DNA was correlated positively with intrahepatic HBV cccDNA (r =0.443,P < 0.01).Intrahepatic HBV cccDNA was highest in patients aged from 31 to 40 years,while serum HBV DNA and intrahepatic total HBV DNA were highest in patients aged between 20 and 30 years.Sixty-four (51.2 %) patients were pathologically confirmed with histological inflammation activity <G2 and with fibrosis score <S2.HBV cccDNA was not correlated with liver inflammation and fibrosis (x2 =0.048,P>0.05).Conclusions In HBV-infected patients with normal liver function,intrahepatic HBV cccDNA is positively correlated with both serum HBV DNA and intrahepatic total HBV DNA.The replication levels of HBV in serum and in liver tissue are different among age groups.There is no correlation between intrahepatic HBV cccDNA and liver pathology.
