目的 探讨基于反求工程技术测量人体股骨头坏死体积的准确性.方法 2011年1月至11月行人工全髋关节置换术治疗16例(20髋)股骨头坏死患者,男10例,女6例;平均年龄为65.7岁(52 ~78岁).股骨头坏死类型:激素性坏死6例(8髋),酒精性坏死4例(5髋),特发性坏死4例(4髋),创伤性坏死2例(3髋).通过以下2种方法测量患者股骨头坏死区域体积:①将股骨头坏死标本的CT图像以.dicom格式导入Mimics 10.1软情,基于图像反求工程技术,重建坏死区域与股骨头的三维图像,利用自行研发的股骨头坏死测量软件,计算所有坏死区域体积与整个股骨头体积;②采用移液法,实体人工测量术后被置换的股骨头体积和坏死区域体积.结果 基于反求工程技术和实体手工测量所得股骨头体积平均分别为(56 125.37 ±4 681.73)、(56 271.05 ±4 539.08) mm3,差异无统计学意义(t=-1.290,P=0.103);股骨头坏死体积平均分别为(25 256.42±3 748.91)、(25 884.68±3 499.75) mm3,差异也无统计学意义(t=-0.750,P=0.504).结论 基于反求工程技术计算机软件能准确测量股骨头坏死区域体积,是一种值得提倡的新方法.

Objective To assess the accuracy of volumetric measurement of femoral head necrosis based on reverse engineering techniques.Methods From January 2011 to November 2011,we treated 16 patients with femoral head necrosis (20 hips).They were 10 men and 6 women,with an average age of 65.7 years.Six patients (8 hips) were afflicted with hormonal necrosis,4 patients (5 hips) with alcoholic necrosis,4 patients (4 hips) with idiopathic necrosis,and 2 patients (3 hips) with traumatic necrosis.The volume of necrosis was measured by the following 2 ways:1) The DICOM CT data of the femoral head necrosis were put into software Mimics 10.1 for 3D reconstruction of the necrotic region and the femoral head using reverse engineering techniques.Next the volumes of necrotic regions and femoral heads were calculated using self-designed software.2) The actual volumes of necrotic regions and femoral heads were calculated manually by displacing them with water.Results The mean volumes of femoral heads measured by computer and by hand were respectively 56,125.37 ±4,681.73 mm3 and 56,271.05 ±4,539.08 mm3; the mean volumes of necrotic regions measured by computer and by hand were respectively 25,256.42 ± 3,748.91 mm3 and 25,884.68 ± 3,499.75 mm3.There were no significant differences between the 2 measurement methods (P >0.05).Conclusion Volumetric measurement of femoral head necrosis based on reverse engineering techniques is accurate and should be recommended.
