目的 探讨声触诊组织量化(virtual touch tissue quantification,VTQ)技术在肝良恶性病灶诊断中的应用价值.方法 对172例肝实性局灶性病变患者共计192枚病灶行VTQ检测,获取病灶及其周围背景肝组织的剪切波速度(shear wave velocity,SWV)值,同时选取60例健康志愿者为对照组.分析不同病理类型病灶SWV的差异,并与背景肝组织、正常肝组织SWV进行比较,采用ROC曲线找出SWV预测肝恶性局灶性病变的最佳截点.结果 肝恶性病灶的SWV值明显高于良性病灶,两者差异具有统计学意义[(2.83±0.91) m/s对(1.83±0.64)m/s,P=0.000];恶性病灶与正常肝组织、良性病灶与正常肝组织间SWV差异均有统计学意义(P<0.01).以SWV=2.19 m/s为截点诊断肝恶性病灶的敏感性、特异性、准确性、阳性预测值、阴性预测值分别为82.5%、75.4%、77.03%、81.16%、78.9%.结论 VTQ技术对肝局灶性病变的良恶性鉴别提供了客观有用的信息,具有良好的临床应用价值.

Objective To explore the clinical application value of virtual touch tissue quantification (VTQ) in the differential diagnosis of focal liver lesions.Methods 174 patients with 194 focal liver lesions and 60 healthy volunteers were enrolled in this study,shear wave velocity(SWV) of lesions,background liver parenchyma and healthy liver were evaluated with VTQ examination.Statistical analysis was performed on the SWV for the differentiation of normal liver,background liver parenchyma and lesions.The cut-off point for SWV was obtained by using ROC curve analysis which was used to predict malignancy lesions.Results The SWV of malignant lesions was significantly higher than that of benign ones [(2.83 ± 0.91)m/s vs (1.83 ± 0.64)m/s,P =0.000].The SWV of healthy liver was significantly lower than both malignant and benign lesions(P <0.01).Based on 2.19m/s as the cut-off point of SWV value,the sensitivity,specificity,accuracy,positive predictive value and negative predictive value for diagnosing malignancies were 82.5%,75.4%,77.03%,81.16%,78.9% respectively.Conclusions VTQ technology could provide complementary information regarding the tissue stiffness,which is useful for the differential diagnosis of focal liver lesions.
