目的 探讨氯氮平与氟哌啶醇治疗对精神分裂症患者体质量、神经肽Y和瘦素的影响及其相互关系.方法 采用随机排列表法,将符合《中国精神障碍分类与诊断标准》第3版精神分裂症诊断标准的60例精神分裂症患者分为氯氮平组和氟哌啶醇组,每组各30例,分别接受氯氮平[250~400(323 ±53) mg/d]和氟哌啶醇[10~20(14±3) mg/d]治疗,并于基线,治疗第3、6周末,测定2组的体质量、血浆神经肽Y和瘦素水平.结果 (1)治疗第3、6周末,氯氮平组体质量[(58.9±10.1)、(61.0±10.3)kg]和神经肽Y[(50.4 ±20.2)、(67.8 ±27.2) ng/L]均高于基线[(57.6±9.9)kg,(39.2±15.7) ng/L],差异有统计学意义(F=9.531,P=0.009;F =37.395,P=0.000;F=7.947,P=0.015;F=22.689,P=0.000),治疗第6周末瘦素[(7.6±4.3)ng/L]显著高于基线[(4.9±2.7) ng/L;F=6.376,P=0.027];氟哌啶醇组各时点体质量(F=0.350,P=0.708)、神经肽Y(F=0.075,P=0.789)和瘦素(F=0.089,P=0.915)比较差异无统计学意义.(2)氯氮平组和氟哌啶醇组各时点体质量(F =6.341,P=0.004)、神经肽Y(F=14.565,P=0.000)和瘦素(F=6.088,P=0.005)存在组别×时间的交互作用.(3)治疗第6周末,氯氮平组体质量变化率与神经肽Y变化率正性相关(r =0.715,P=0.006).结论 氯氮平治疗过程中伴有神经肽Y、瘦素升高和体质量增加,且神经肽Y的升高与体质量增加相关.

Objective To investigate the changes of neuropeptide Y(NPY),leptin,body weight and their relationship in schizophrenics with clozapine and haloperidol treatment.Methods Thirty schizophrenic patients treated with clozapine [250-400 (323 ± 53) mg/d] and 30 patients treated with haloperidol [10-20 (14 ± 3) mg/d] were involved in the study.Body weight and plasma NPY,leptin levels were measured at weeks 0 and 3,6.Results (1) The body weight and plasma NPY levels in clozapine group increased significantly at 3 weekend[(58.9 ± 10.1) kg,(50.4 ±20.2) ng/L] and 6 weekend [(61.0 ± 10.3) kg,(67.8 ± 27.2) ng/L] compared to those before the treatment [(57.6 ± 9.9) kg,(39.2 ± 15.7) ng/L](F=9.531,P=0.009;F=37.395,P=0.000;F=7.947,P=0.015;F=22.689,P=0.000),while leptin levels increased significantly at 6 weekend [(7.6 ±4.3) ng/L] compared to baseline [(4.9 ±2.7)ng/L;F =6.376,P =0.027].The differences on body weight(F =0.350,P =0.708) and plasma NPY (F =0.075,P =0.789),leptin levels (F =0.089,t =0.915) at each timepoint in haloperidol group were not significant.(2) The altering tendency of body weight (F = 6.341,P =0.004) and plasma NPY (F =14.565,P =0.000),leptin levels (F =6.088,P =0.005) at each timepoint were significantly different in both groups,there were interactions between groups and time.(3) In clozapine group,the altering rate of body weight at 6th weekend was positively correlated with that of plasma NPY levels (r =0.715,P =0.006).Conclusion The data indicates that NPY and leptin be elevated and body weight increased in the treatment with clozapine,moreover,the increasing of NPY have a significant relationship with that of body weight.
