目的 探讨6～10岁儿童第一恒磨牙患龋与乳磨牙患龋的相关性,以期为临床提供参考.方法 采用整群、分层、随机抽样的方法,抽取济南市10个区(县)40所小学,对6 ～ 10岁符合纳入标准的3680名儿童进行患龋情况调查,并分析第一恒磨牙患龋率与年龄、性别及乳磨牙患龋之间的相关性,对第一恒磨牙龋失补牙数(decayed-missed-filled teeth,DMFT)、龋失补牙面数(decayed-missed-filled surfaces,DMFS)及乳磨牙龋失补牙数(decayed-missed-filled teeth,dmft)、龋失补牙面数(decayed-missed-filled surfaces,dmfs)进行相关分析.结果 6～10岁第一恒磨牙DMFT为1440,龋均0.39(1440/3680);患龋率20.7％ (763/3680),女性患龋率[28.9％ (509/1762)]显著高于男性[13.2％ (254/1918)] (P ＜0.01)；第一恒磨牙患龋率随年龄增长呈上升趋势.第一恒磨牙DMFT与乳牙dmft呈显著正相关关系(r=0.31,P＜0.01),DMFS与dmfs呈显著正相关(r=0.35,P＜0.01).当dmft＞3、dmfs＞ 19时第一恒磨牙患龋率成倍增长.结论 第一恒磨牙患龋率与性别、年龄及乳磨牙患龋指数有关,对乳磨牙高龋患者应早期对第一恒磨牙采取必要的防龋措施.

목적 탐토6～10세인동제일항마아환우여유마아환우적상관성,이기위림상제공삼고.방법 채용정군、분층、수궤추양적방법,추취제남시10개구(현)40소소학,대6 ～ 10세부합납입표준적3680명인동진행환우정황조사,병분석제일항마아환우솔여년령、성별급유마아환우지간적상관성,대제일항마아우실보아수(decayed-missed-filled teeth,DMFT)、우실보아면수(decayed-missed-filled surfaces,DMFS)급유마아우실보아수(decayed-missed-filled teeth,dmft)、우실보아면수(decayed-missed-filled surfaces,dmfs)진행상관분석.결과 6～10세제일항마아DMFT위1440,우균0.39(1440/3680);환우솔20.7％ (763/3680),녀성환우솔[28.9％ (509/1762)]현저고우남성[13.2％ (254/1918)] (P ＜0.01)；제일항마아환우솔수년령증장정상승추세.제일항마아DMFT여유아dmft정현저정상관관계(r=0.31,P＜0.01),DMFS여dmfs정현저정상관(r=0.35,P＜0.01).당dmft＞3、dmfs＞ 19시제일항마아환우솔성배증장.결론 제일항마아환우솔여성별、년령급유마아환우지수유관,대유마아고우환자응조기대제일항마아채취필요적방우조시.
Objective To investigate the association of first permanent molars caries prevalence with primary molar caries experience.Methods Cluster,stratified and random sampling method,was used and 3680 children 6-to 10-year-old were randomizly selected from primary schools of Jinan city.Caries status of first permanent molars,decayed-missed-filled surfaces (DMFS,dmfs),decayed-missed-filled teeth (DMFT,dmft),were examined.Chi-square test was used to analyze the relationship among the caries incidence and the age,sex and to analyze the relationship between the first permanent molars DMFT,DMFS and the primary molars dmft,dmfs.Results Twenty point seven percent(763/3680) of children had first permanent molars caries,significantly higher(P ＜ 0.01) in girls [28.9％ (509/1762)] than in boys [13.2％ (254/1918)],a positive correlation (r =0.31,P ＜ 0.01) was found between caries experiences of primary molars (dmft) and caries of first permanent molars (DMFT).Higher permanent molar caries was found in subjects with dmft higher than 3 or dmfs higher than 19.Conclusions Sex,age and the caries experience of primary molars were factors closely related with the caries rate of the first permanent molars.Enhanced prevention is suggested on the first permanent molars of children who has higher prevalence of caries in primary molars.
