目的探讨单侧唇裂继发鼻畸形修复术中,用自体鼻中隔软骨重建鼻软骨性支架结构的可行性.方法 2010年1月至2011年10月北京大学口腔医学院·口腔医院口腔颌面外科行鼻整形术的单侧唇裂继发鼻畸形患者41例.术中行横跨鼻小柱切口和双侧软骨下切口,内侧脚间切开,显露切取偏曲的鼻中隔软骨,以之为材料修复重建鼻支架结构,整复鼻外形和鼻中隔偏曲.术前术后摄取正侧位面像和头颅CT检查,评价鼻形态变化和鼻中隔偏曲情况.结果 41例单侧唇裂继发鼻畸形患者通过手术,鼻形态明显改善,术后主观评价手术前后分值平均为2.80;CT扫描显示鼻中隔偏曲程度30例(73％)矫治效果有效,11例(27％)矫治无效,即鼻中隔偏曲程度无改善.结论以自体鼻中隔软骨为材料重建鼻软骨性支架结构可有效改善单侧唇裂继发鼻畸形,并同期矫治鼻中隔软骨性偏曲.
목적 탐토단측진렬계발비기형수복술중,용자체비중격연골중건비연골성지가결구적가행성.방법 2010년1월지2011년10월북경대학구강의학원·구강의원구강합면외과행비정형술적단측진렬계발비기형환자41례.술중행횡과비소주절구화쌍측연골하절구,내측각간절개,현로절취편곡적비중격연골,이지위재료수복중건비지가결구,정복비외형화비중격편곡.술전술후섭취정측위면상화두로CT검사,평개비형태변화화비중격편곡정황.결과 41례단측진렬계발비기형환자통과수술,비형태명현개선,술후주관평개수술전후분치평균위2.80;CT소묘현시비중격편곡정도30례(73％)교치효과유효,11례(27％)교치무효,즉비중격편곡정도무개선.결론 이자체비중격연골위재료중건비연골성지가결구가유효개선단측진렬계발비기형,병동기교치비중격연골성편곡.
Objective To investigate the result of the correction of secondary deformities in the unilateral cleft lip patients with autologous septal cartilage.Methods A total of 41 unilateral cleft lip patients who underwent rhinoplasty were included in this study,including 15 cleft lip and 26 cleft lip and palate patients.In the rhinoplasty,the autologous septal cartilage was harvested and trimmed.Then one long piece of septal cartilage graft was used as columellar strut,and others as onlay grafts on the tip,dorsal side of the nose,and the lower and/or upper lateral cartilage of the cleft side.The results were assessed by X-ray.Results The appearance of the nose apparantly improved after the rhinoplasty.The deviation of the cartilaginous septum was corrected in 31 patients.Ten cases with severe bony septum deviation had no improvement.The subjective evaluation scores improved an average of 2.80 after the rhinoplasty.Conclusions With the rhinoplasty presented,we can reconstruct the cartilaginous bracket of the unilateral cleft lip nose with autologous septal cartilage and improve the appearance.The cartilaginous deviation also can be corrected.
