将493例2型糖尿病患者按照是否服用二甲双胍分为双胍组和非双胍组,比较2组患者血清癌胚抗原(CEA)水平和生化特征,采用Pearson或Spearman相关分析和多元逐步回归分析影响CEA水平的因素.结果显示双胍组血清CEA水平明显低于非双胍组[(2.57±1.47对3.09±1.79)mmol/L,P<0.01];女性血清CEA水平明显低于男性[(2.48±1.41对3.12±1.78)mmol/L,P<0.01],绝经后高于绝经前(P<0.05).Spearman相关分析显示CEA与HbA1C、糖化血清白蛋白、应用二甲双胍、性别和睾酮呈显著相关(P<0.01).多元逐步回归分析显示HbA1C(P<0.01)、二甲双胍(P=0.019)、性别(P<0.01)和睾酮(P<0.01)是CEA的独立影响因素.提示2型糖尿病患者血清CEA水平存在性别差异,二甲双胍可降低2型糖尿病患者,尤其是女性患者的血清CEA水平.

Serum carcino embryonic antigen (CEA) levels were measured and compared in 493 patients with type 2 diabetes including one group receiving,and another group without metformin treatment.Associations between CEA and other variables were assessed with Spearman or Pearson correlation analysis and multiple stepwise regression analysis.The results showed that CEA level in metformin group was significantly lower than that in nonmetformim group[(2.57 ± 1.47 vs 3.09 ± 1.79) mmol/L,P<0.01] ; CEA level in females was significantly lower than that of males [(2.48 ± 1.41 vs 3.12 ± 1.78) mmol/L,P<0.01] ; CEA level in postmenopausal women was significantly higher than that in premenopausal women (P<0.05).The correlation analysis showed that serum CEA levels were significantly associated with HbA1C,glycated albumin,metformin,gender,and testosterone (P <0.01).Multiple stepwise regression analysis further revealed that HbA1C(P<0.01),metformin administration (P =0.019),gender(P<0.01),and T levels (P<0.01) were independent influencing factors of CEA concentrations.These results suggest that there is gender difference in serum CEA levels in type 2 diabetic patients.Metformin may reduce serum CEA levels in type 2 diabetic patients,especially in the female patients.
