目的 观察甲状旁腺素(PTH)对老年小鼠骨折后骨愈合的作用.方法 12月龄野生型小鼠采用三点加压法制造右侧股骨稳定性骨折模型,术后分为对照组(6只,生理盐水皮下注射0.1 ml/d)和PTH干预组(6只,rhPTH 1-34皮下注射40μg·kg-1·d-1),14 d后血管造影并处死动物,取材右侧股骨中段,固定、micro CT扫描、包埋切片,血管内皮细胞生长因子(VEGF)抗体免疫组织化学染色,图像分析统计软件对骨痂各部位VEGF表达进行统计分析.结果 与对照组相比,PTH干预14 d显著性增加骨痂部位骨容积和骨量(P<0.05),升高单位体积骨小梁数量,降低骨小梁分离度(P<0.05);增加骨痂部位血管发生,尤其管径≤50μm血管显著性增加(P<0.05);PTH干预组骨痂内骨小梁、软骨和间质部位VEGF表达量显著性增加(P<0.05).结论 PTH促进骨痂内软骨发生和骨发生,并且促进骨痂内血管发生,可能是通过上调骨痂内VEGF的表达起作用.

목적 관찰갑상방선소(PTH)대노년소서골절후골유합적작용.방법 12월령야생형소서채용삼점가압법제조우측고골은정성골절모형,술후분위대조조(6지,생리염수피하주사0.1 ml/d)화PTH간예조(6지,rhPTH 1-34피하주사40μg·kg-1·d-1),14 d후혈관조영병처사동물,취재우측고골중단,고정、micro CT소묘、포매절편,혈관내피세포생장인자(VEGF)항체면역조직화학염색,도상분석통계연건대골가각부위VEGF표체진행통계분석.결과 여대조조상비,PTH간예14 d현저성증가골가부위골용적화골량(P<0.05),승고단위체적골소량수량,강저골소량분리도(P<0.05);증가골가부위혈관발생,우기관경≤50μm혈관현저성증가(P<0.05);PTH간예조골가내골소량、연골화간질부위VEGF표체량현저성증가(P<0.05).결론 PTH촉진골가내연골발생화골발생,병차촉진골가내혈관발생,가능시통과상조골가내VEGF적표체기작용.
Objective To investigate the effect of parathyroid hormone (PTH) on fracture healing in old mice.Method 12-month old mice were made models of stabilized fracture of right femur by three point pressing machine,then these mice were separated into two groups:CON group (n =6) with saline injected subcutaneously once daily and PTH group (n =6) with rhPTH 1-34 40 μg/kg injected subcutaneously once daily.Mice were perfused before sacrificed on 14 th day after fracture.Right femur were isolated to be fixed and decalcified,and scanned by micro CT,and then with vascular endothelial growth factor (VEGF) expression in callus tested by immunohistochemistry.Result Compared to CON group,in PTH group there were significantly increased callus bone mass and callus bone volume (P<0.05),and also significantly increased callus trabecular bone number and decreased trabecular bone separation (P<0.05) ; in addition,in PTH group there were enhanced vascular volume and tube formation in callus,especially the small vessels with diameter≤50 μm (P<0.05),and upregulated VEGF expression in trabecular bone,cartilage and mesenchyme in callus (P<0.05).Conclusion PTH promoted chondrogenesis,osteogenesis,and angiogenesis of callus at least partially through promoting VEGF expression in callus.
