目的 分析以头痛为首发症状的颅内静脉窦血栓形成(CVST)的头痛特点及其与脑脊液压力及静脉窦血栓部位的关系.方法 收集以头痛为首发症状并经数字减影血管造影(DSA)或头颅磁共振静脉成像(MRV)证实为CVST的117例患者的病历资料进行回顾性分析.结果 117例CVST患者中全头痛和非全头痛者分别占65.8%(77例)和34.2%(40例);脑脊液压力增高、正常和减者低分别占79.5%(93例)、7.7%(9例)和12.8%(15例);多静脉窦、双静脉窦和单静脉窦血栓形成者分别占44.4%(52例)、36.7%(43例)和18.8%(22例).全头痛组脑脊液压力升高、正常、减低的构成比分别为75.3% (58/77)、9.1% (7/77)、15.6% (12/77),非全头痛组分别为87.5% (35/40)、5.0% (2/40)、7.5% (3/40),2组脑脊液压力构成比的差异无统计学意义(P>0.05).全头痛组与非全头痛组CVST部分分布差异无统计学意义(P>0.05).结论 头痛为首发症状的CVST以全头痛为主,以多静脉窦血栓形成多见,多伴有颅内压增高,但对于颅内压正常尤其颅内压明显减低的头痛患者,也应注意CVST的可能.

Objective To analyze the correlation of the characteristics of headache and intracranial pressure and the site of venous sinus thrombosis in cerebral venous sinus thrombosis (CVST) patients.Methods A retrospective review of 117 CVST patients data was performed.All patients were diagnosed by digital subtraction angiography (DSA) or magnetic resonance venography (MRV).And they were divided into full headache group (77 cases) and part headache group (40 cases).Intracranial pressure and the site of thrombosis of all CVST patients were reviewed.Results ①The percentage of full headache was 65.8% (77/117).The percentage of high,normal and low intracranial pressure were 79.5% (93/117),7.7% (9/117) and 12.8% (15/117) respectively.And the percentage of multiple venous sinus,double venous sinus and single venous sinus were 44.4% (52/117),36.7% (43/117) and 18.8% (22/117) respectively.②The high intracranial pressure in full headache group is higher than the part headache group.The normal intracranial pressure in full headache group is nearly same as the part headache group.The low intracranial pressure in full headache group is less than the part headache group.But there are no significant difference between groups (P > 0.05).③Incidence rate of single venous sinus thrombosis in full headache group is slightly higher than the part headache group.But incidence rate of double and multiple venous sinus thrombosis in full headache group is slightly lower than the part headache group.But there is no significant difference between groups (P > 0.05).Conclusions CVST patients with headache as the intial symptom mainly manifest as the full headache,multiple venous sinus thrombosis,and intracranial hypertension.If headache patients with normal intracranial pressure or intranial hypotension were encountered,we should pay more attention to the diagnosis of CVST.
