目的 探讨18F-氟代脱氧葡萄糖正电子发射计算机断层扫描-电子计算机体层成像(18F-FDGPET-CT)双时相显像在肝脏恶性肿瘤诊断中的临床应用价值.方法 经病理及随访证实的88例肝脏恶性肿瘤患者(原发性肝癌50例,胆管细胞癌5例,肝癌术后复发11例,肝癌移植术后复发5例,肝转移瘤12例,肝母细胞瘤5例),均行双时相显像.早期显像在注射FDG后1h进行,延迟显像在注药后2~4h进行.对双时相显像结果采用阅片目测定性和半定量方法分析,勾画出感兴趣区(ROI)后,根据受检者体重、注射药物剂量及时间自动生成常规及延迟标准摄取值最大值(SUVmax)和平均值(SUVmean),并计算2次显像SUVmax和SUVmean.结果 经过18F-FDGT PET-CT早期及延迟显像,共发现598个放射性异常浓聚灶,经病理或临床随访证实,88例患者18F-FDG PET-CT显像中,常规PET-CT显像诊断肝恶性肿瘤阳性率为73.9% (65/88);双时相显像诊断阳性率为90.9% (80/88).48例原发性肝癌、5例胆管细胞癌、8例肝癌术后复发、9例肝转移瘤、5例肝母细胞瘤术后、5例肝癌移植术后复发者18F-FDG异常高摄取.结论 18F-FDG PET-CT双时相显像可提高肝癌诊断阳性率.

목적 탐토18F-불대탈양포도당정전자발사계산궤단층소묘-전자계산궤체층성상(18F-FDGPET-CT)쌍시상현상재간장악성종류진단중적림상응용개치.방법 경병리급수방증실적88례간장악성종류환자(원발성간암50례,담관세포암5례,간암술후복발11례,간암이식술후복발5례,간전이류12례,간모세포류5례),균행쌍시상현상.조기현상재주사FDG후1h진행,연지현상재주약후2~4h진행.대쌍시상현상결과채용열편목측정성화반정량방법분석,구화출감흥취구(ROI)후,근거수검자체중、주사약물제량급시간자동생성상규급연지표준섭취치최대치(SUVmax)화평균치(SUVmean),병계산2차현상SUVmax화SUVmean.결과 경과18F-FDGT PET-CT조기급연지현상,공발현598개방사성이상농취조,경병리혹림상수방증실,88례환자18F-FDG PET-CT현상중,상규PET-CT현상진단간악성종류양성솔위73.9% (65/88);쌍시상현상진단양성솔위90.9% (80/88).48례원발성간암、5례담관세포암、8례간암술후복발、9례간전이류、5례간모세포류술후、5례간암이식술후복발자18F-FDG이상고섭취.결론 18F-FDG PET-CT쌍시상현상가제고간암진단양성솔.
Objective To evaluate the clinical diagnostic value of dual-phase 18F-FDG PET-CT for detecting the hepatic malignant lesions.Methods All 88 hepatic malignant neoplasms were enrolled in this study.Of these 88 cases,there were 50 cases with primary hepatocellula carcinoma,5 cases with intrahepatic cholangiocarcinoma,16 cases with postoperative recurrence,5 cases with hepatoblastom and 12 cases with metastasis.All patients underwent dual-phase 18F-FDG PET-CT.The early images were obtained at 1 hour after 18F-FDG injection,and delayed images at 2-4 hours.For all lesions,the results were analyzed by qualitative and semiquantitative methods.SUVmax,SUVmean and SUV delayed of tumor were calculated.Results After early and delayed imaging of 18F-FDGT PET-CT,a total of 598 radioactive lesions had abnormal uptake,after confirmed by pathology or clinical follow-up,among 88 patients with 18F-FDG PET-CT imaging,conventional PET-CT imaging diagnosis of hepatic tumor-positive rate was 73.9% (65/88) ; dual-phase imaging diagnostic positive rate was 90.9% (80/88).48 cases of primary liver cancer,five cases of cholangiocarcinoma,eight cases of liver cancer recurrence,9 cases with liver metastases,five cases of hepatoblastoma after surgery and five cases of liver transplantation recurrence had abnormally high intake by 18 F-FDG.Conclusion Combining early imaging with delayed imaging can provide a higher diagnostic sensitivity than the early imaging.
