目的 探讨血清可溶性白细胞介素2受体(sIL-2R)和可溶性肿瘤坏死因子受体Ⅱ(sTNFRⅡ)水平在判断肺结节病活动性中的应用价值.方法 52例经病理确诊的肺结节病患者中,活动期28例,非活动期24例.用酶联免疫吸附试验(ELISA)法检测其外周血血清中sIL-2R、sTNFRⅡ,用紫外分光光度法测定血管紧张素转换酶(ACE)的水平,并与30例健康志愿者(对照组)对比.结果 活动期肺结节病患者血清中sIL-2R、sTNFRⅡ水平分别为(1.3±0.6)、(0.95±0.29) μg/L,明显高于对照组、非活动期肺结节病组[分别为(0.9±0.4)、(0.71 ±0.21) μg/L及(0.9±0.4)、(0.77±0.32)μg/L],差异均有统计学意义(均P<0.05);对照组与非活动期肺结节病组患者血清中sIL-2R、sTNFRⅡ水平差异无统计学意义(P>0.05).活动期肺结节病患者血清中ACE与sIL-2R水平呈正相关(r=0.869,P<0.01),与sTNFRⅡ水平无相关性(r=0.103,P>0.05).结论 血清中sIL-2R及sTNFRⅡ可能作为评价肺结节病活动性的指标,以指导临床治疗.

목적 탐토혈청가용성백세포개소2수체(sIL-2R)화가용성종류배사인자수체Ⅱ(sTNFRⅡ)수평재판단폐결절병활동성중적응용개치.방법 52례경병리학진적폐결절병환자중,활동기28례,비활동기24례.용매련면역흡부시험(ELISA)법검측기외주혈혈청중sIL-2R、sTNFRⅡ,용자외분광광도법측정혈관긴장소전환매(ACE)적수평,병여30례건강지원자(대조조)대비.결과 활동기폐결절병환자혈청중sIL-2R、sTNFRⅡ수평분별위(1.3±0.6)、(0.95±0.29) μg/L,명현고우대조조、비활동기폐결절병조[분별위(0.9±0.4)、(0.71 ±0.21) μg/L급(0.9±0.4)、(0.77±0.32)μg/L],차이균유통계학의의(균P<0.05);대조조여비활동기폐결절병조환자혈청중sIL-2R、sTNFRⅡ수평차이무통계학의의(P>0.05).활동기폐결절병환자혈청중ACE여sIL-2R수평정정상관(r=0.869,P<0.01),여sTNFRⅡ수평무상관성(r=0.103,P>0.05).결론 혈청중sIL-2R급sTNFRⅡ가능작위평개폐결절병활동성적지표,이지도림상치료.
Objective To detect serum soluble inerleukin-2 receptor (sIL-2R),soluble tumor necrosis factor receptor Ⅱ (sTNFR Ⅱ) and angiotensin converting enzyme (ACE) levels in active pulmonary sarcoidosis patients and normal human; to evaluate the value of sIL-2R,sTNFR Ⅱ in pulmonary sarcoidosis activity.Methods Of 52 cases with pulmonary sarcoidosis,28 cases were in active period,24 cases were in inactive period.Serum sIL-2R and sTNFR Ⅱ were detected by enzyme linked immunosorbent assay.Serum ACE level was determined by ultraviolet spectrophotometry and compared with 30 healthy volunteers.Results The serum sIL-2R and sTNFR Ⅱ of patients with active pulmonary sarcoidosis were (1.3 ± 0.6),(0.95 ± 0.29) μg/L and significantly higher than those in control group and patients with inactive sarcoidosis [(0.9 ± 0.4),(0.71 ± 0.21) μg/L and (0.9 ± 0.4),(0.77 ± 0.32) μg/L,respectively],and there were significant differences (P < 0.05).There was no significant difference between patients with sarcoidosis in the inactive period and healthy volunteers(P >0.05).There was a correlation between the level of ACE and sIL-2R in patients with active pulmonary sarcoidosis (r =0.869,P < 0.01),but there was no correlation between the levels of ACE and sTNFR Ⅱ (r =0.103,P > 0.05).Conclusion The sIL-2R and sTNFR Ⅱ levels may serve as the indexes of sarcoidosis activity in order to guide the clinical treatment.
