目的 探讨非酒精性脂肪性肝病患者血清脂联素与胰岛素表达水平及其相关性.方法 选择2010年1月至2011年1月自贡市仁济医学中心收治的非酒精性脂肪性肝病患者50例为观察组,同时收集健康志愿者40例为对照组.用酶联免疫吸附测定法检测2组空腹及餐后2h血清脂联素水平,用化学发光法检测空腹及餐后2h胰岛素表达水平,并进行相关性分析.结果 观察组和正常对照组空腹脂联素水平分别为(912 ±29)、(1 033±53)μg/L,组间差异有统计学意义(P<0.05);空腹胰岛素水平分别为(13.8±1.0)、(7.3±1.0)mU/L,组间差异有统计学意义(P<0.01).观察组和正常对照组餐后2h脂联素水平分别为(854 ±38)、(1 010±48)μg/L,组间差异有统计学意义(P<0.05);餐后2h胰岛素水平分别为(112 ±23)、(28 ±3) μg/L,组间差异有统计学意义(P<0.01).相关性分析显示,非酒精性脂肪性肝病患者空腹及餐后2h脂联素与胰岛素水平均呈负相关(空腹:r=-0.328,P=0.020;餐后2 h:r=-0.391,P =0.005).结论 非酒精性脂肪性肝病进展的同时伴随着胰岛素及脂联素水平的变化,二者呈负相关,这种负相关可能以餐后2h更明显.

목적 탐토비주정성지방성간병환자혈청지련소여이도소표체수평급기상관성.방법 선택2010년1월지2011년1월자공시인제의학중심수치적비주정성지방성간병환자50례위관찰조,동시수집건강지원자40례위대조조.용매련면역흡부측정법검측2조공복급찬후2h혈청지련소수평,용화학발광법검측공복급찬후2h이도소표체수평,병진행상관성분석.결과 관찰조화정상대조조공복지련소수평분별위(912 ±29)、(1 033±53)μg/L,조간차이유통계학의의(P<0.05);공복이도소수평분별위(13.8±1.0)、(7.3±1.0)mU/L,조간차이유통계학의의(P<0.01).관찰조화정상대조조찬후2h지련소수평분별위(854 ±38)、(1 010±48)μg/L,조간차이유통계학의의(P<0.05);찬후2h이도소수평분별위(112 ±23)、(28 ±3) μg/L,조간차이유통계학의의(P<0.01).상관성분석현시,비주정성지방성간병환자공복급찬후2h지련소여이도소수평균정부상관(공복:r=-0.328,P=0.020;찬후2 h:r=-0.391,P =0.005).결론 비주정성지방성간병진전적동시반수착이도소급지련소수평적변화,이자정부상관,저충부상관가능이찬후2h경명현.
Objective To investigate the expression level of adiponectin and insulin in non-alcoholic fatty liver disease(NAFLD) and explore the correlation between adiponectin and insulin.Methods Totally 50 NAFLD cases were as observation group and contemporary 40 normal were as control group; the adiponectin of fasting and postprandial 2 hours of 50 NAFLD cases and contemporary 40 normal control was determined by enzyme linked immunosorbent assay(ELISA) ; insulin of fasting and postprandial 2 hour was determined by chemi-luminescence.The correlation between the two factors of fasting and postprandial 2 hour was analyzed.Results There was a statistical significant expression of fasting serum adiponectin in observation group [(912 ± 29) μg/L vs (1 033 ± 53)μg/L,P < 0.05].There was a statistical difference of insulin of fasting between observation group and control group [(13.8 ± 1.0) mU/ L vs (7.3 ± 1.0) mU/L,P <0.01].There was a statistical significance of expression of postprandial 2 hour serum adiponectin in observation group [(854 ± 38) μg/L vs (1 010 ± 48) μg/L,P < 0.05].There was a statistical difference of insulin of postprandial 2 hour between observation group and control group[(112 ± 23)μg/L vs (28 ± 3)μg/L,P < 0.01].The expression level of adiponectin and insulin in either fasting or postprandial 2 hour had a negative correlation(fasting r =-0.328,P =0.02 ; postprandial 2 hour r =-0.391,P =0.005).Conclusion With the NAFLD progressing,there is a negative correlation between insulin and adiponectin expression level.
