目的 探讨成人松果体实质肿瘤的手术疗效.方法 回顾性分析北京天坛医院神经外科2007年1月至2013年10月收治了39例成人(≥18岁)松果体实质细胞肿瘤的临床资料并进行随访.结果 肿瘤全切除19例,次全切除12例,部分切除6例,仅行肿瘤活检2例,无手术死亡病例.术后病理结果:松果体细胞瘤14例,中分化松果体实质肿瘤15例,松果体母细胞瘤10例.随访到的32例患者中,5例患者因肿瘤复发而死亡,中分化和松果体母细胞瘤中位生存期分别为34.5个月和29.8个月.结论 松果体细胞瘤手术治疗效果良好,中分化松果体实质肿瘤和松果体母细胞瘤治疗应首选手术,放化疗的效果尚待确定.
目的 探討成人鬆果體實質腫瘤的手術療效.方法 迴顧性分析北京天罈醫院神經外科2007年1月至2013年10月收治瞭39例成人(≥18歲)鬆果體實質細胞腫瘤的臨床資料併進行隨訪.結果 腫瘤全切除19例,次全切除12例,部分切除6例,僅行腫瘤活檢2例,無手術死亡病例.術後病理結果:鬆果體細胞瘤14例,中分化鬆果體實質腫瘤15例,鬆果體母細胞瘤10例.隨訪到的32例患者中,5例患者因腫瘤複髮而死亡,中分化和鬆果體母細胞瘤中位生存期分彆為34.5箇月和29.8箇月.結論 鬆果體細胞瘤手術治療效果良好,中分化鬆果體實質腫瘤和鬆果體母細胞瘤治療應首選手術,放化療的效果尚待確定.

Objective To study the clinical characteristics,surgical treatment and outcome of pineal parenchymal tumors(PPT) in adult patients.Methods The adult patients (≥18 years old) with PPT diagnosed between 2007 and 2013 were retrospectively studied and their surgical results were followed-up.Results 37 PPT tumors were surgically removed,including 19 total removal,12 subtotal removal and 6 partial removal.The biopsy was made in 2 patients.The pathological type of 14 cases was pineocytoma,15 pineal parenchymal tumors of intermediate differentiation,and 10 pinealoblastoma.In 32 patients of follow-up,there were 5 patients died because of tumor recurrence.The median overall survival was 34.5 months for intermediate differentiation and 29.8 months for pinealoblastoma.Conclusions Pineocytomas' patients have good survival prognosis after surgical treatment.Microsurgery is the most effective therapy for pineal parenchymal tumor of intermediate differentiation and pineoblastoma.The role of adjuvant radiotherapy and chemotherapy remains to be defined.
