目的 分析术前模拟翼点入路3D-DSA图像对开颅夹闭颅内动脉瘤手术的指导意义.方法 以45例蛛网膜下腔出血颅内动脉瘤并且行开颅夹闭手术患者为研究对象,术前行目标血管旋转造影,数据进行重建,并旋转重建图像至模拟翼点入路.观察并测量模拟翼点入路3D-DSA图像的相关信息和数据,指导开颅夹闭动脉瘤手术.观察和测量开颅手术过程中相关信息和数据,将两者进行比较.结果 术前模拟翼点入路3D-DSA图像测量数据与开颅手术过程中测量的数据相符.术前对于动脉瘤夹选择的预估亦与术中实际情况高度相符.结论 通过术前旋转3D-DSA重建图像至模拟翼点入路体位,可使手术者获得大量信息,对于提高手术成功率,降低手术并发症发生,改善患者预后有重要意义.

목적 분석술전모의익점입로3D-DSA도상대개로협폐로내동맥류수술적지도의의.방법 이45례주망막하강출혈로내동맥류병차행개로협폐수술환자위연구대상,술전행목표혈관선전조영,수거진행중건,병선전중건도상지모의익점입로.관찰병측량모의익점입로3D-DSA도상적상관신식화수거,지도개로협폐동맥류수술.관찰화측량개로수술과정중상관신식화수거,장량자진행비교.결과 술전모의익점입로3D-DSA도상측량수거여개로수술과정중측량적수거상부.술전대우동맥류협선택적예고역여술중실제정황고도상부.결론 통과술전선전3D-DSA중건도상지모의익점입로체위,가사수술자획득대량신식,대우제고수술성공솔,강저수술병발증발생,개선환자예후유중요의의.
Objective To analyze the guiding significance of 3D-DSA images of simulated pterional approach before craniotomy clipping surgery of intracranial aneurysms.Methods 45 cases of intracranial aneurysms and craniotomy clipping surgery patients were included in this study.The data of preoperative rotating angiography to target vessels were transferred to AW4.5 computer workstation for data reconstruction.The reconstructed images were rotated to simulated pterional approach.The relevant information and data of the 3D-DSA images of simulated pterional approach were observed and measured to guide intracranial aneurysm clipping surgery and were compared with the data of surgery.Results The information and data in the 3D-DSA images of simulated pterional approach was accord with the actual condition in operation.The preoperative choice of aneurysm clips was in accord with that during operation.Conclusions Rotating the 3D-DSA images to simulated pterional approach before the operation could make the surgeon to obtain a lot of information,and had important significance to improve the success rate of operation,and to reduce operation complication rate to improve the prognosis of patients.
