目的 观察重症急性胰腺炎(SAP)时大鼠腺垂体促甲状腺素细胞超微结构和功能的变化.方法 将雄性Wistar大鼠40只随机分为5组:假手术组(SO组)及SAP 1、3、6、12h组.胆胰管逆行注射5%牛磺胆酸钠制备SAP模型.测定血清淀粉酶、脂肪酶酶、总三碘甲腺原氨酸(T3)、总甲状腺素(T4)、游离T3(FT3)、游离T4(FT4)、促甲状腺素(TSH)水平.光镜观察胰腺及垂体病理改变,电镜观察促甲状腺素细胞超微结构改变.结果 SAP组T3(0.46±0.09) nmol/L、FT3(0.93±0.07) nmol/L及TSH(0.063 ±0.012) nmol/L水平均较SO组[分别为(0.98±0.11)、(2.42±0.35)、(0.063 ±0.012) nmol/L]明显下降(P<0.05).T4及FT4水平无明显变化.光镜下可见垂体组织损伤.电镜下见促甲状腺素细胞各细胞器出现结构破坏,分泌颗粒减少.结论 在SAP时,大鼠腺垂体促甲状腺素细胞出现功能及超微结构改变,SAP大鼠垂体-甲状腺功能变化可能与此改变相关.

목적 관찰중증급성이선염(SAP)시대서선수체촉갑상선소세포초미결구화공능적변화.방법 장웅성Wistar대서40지수궤분위5조:가수술조(SO조)급SAP 1、3、6、12h조.담이관역행주사5%우광담산납제비SAP모형.측정혈청정분매、지방매매、총삼전갑선원안산(T3)、총갑상선소(T4)、유리T3(FT3)、유리T4(FT4)、촉갑상선소(TSH)수평.광경관찰이선급수체병리개변,전경관찰촉갑상선소세포초미결구개변.결과 SAP조T3(0.46±0.09) nmol/L、FT3(0.93±0.07) nmol/L급TSH(0.063 ±0.012) nmol/L수평균교SO조[분별위(0.98±0.11)、(2.42±0.35)、(0.063 ±0.012) nmol/L]명현하강(P<0.05).T4급FT4수평무명현변화.광경하가견수체조직손상.전경하견촉갑상선소세포각세포기출현결구파배,분비과립감소.결론 재SAP시,대서선수체촉갑상선소세포출현공능급초미결구개변,SAP대서수체-갑상선공능변화가능여차개변상관.
Objective To investigate ultrastructural and functional changes of the thyrotroph cells in rat severe acute pancreatitis (SAP) model.Methods Forty Wistar rats were randomly divided into 5 groups:the sham operation group (SO group,n =8),SAP 1,3,6,and 12 h groups (n =8 each).SAP model was induced by retrograde injection of 5% sodium taurocholate into the biliopancreatic duct.Serum levels of amylase,lipase,thyroxine ( T4 ),triiodothyronine ( T3 ),free T3 ( FT3 ),free T4 ( FT4 ) and thyroid stimulating hormone (TSH) were measured.Pancreas and pituitary tissues were taken for pathological examination under a light microscope.Thyrotroph cells were observed under the electronic microscopy.Results SAP model was successfully established.T3 (0.46 ± 0.09 ) nmol/L,FT3 (0.93 ± 0.07 ) nmol/Land TSH (0.063 ±0.012) nmol/L levels were reduced in SAP groups as compared with SO group [ T3:(0.98±0.11) nmol/L; FT3:(0.93 ±0.07) nmol/L,TSH:(0.063 ±0.012) nmol/L,P<0.05].There was no significant difference in T4 and FT4 levels between SAP groups and SO group.Pituitary interstitial congestion,basophilic cell swelling and partial necrosis were observed under a light microscope.With the time over,reduced secretory granule and organelles destruction in thyrotroph cells were observed under the electronic microscopy.Conclusion Significant changes of ultrastructure and function were observed in thyrotroph cells in SAP,which might be contributed to the pituitary-thyroid dysfunction in SAP rat model.
