目的 探讨星形细胞上调基因-1(AEG-1)和血管内皮生长因子(VEGF)在胆管癌中的表达水平及临床意义.方法 用免疫组织化学链霉菌抗生物素蛋白-过氧化物酶(SP)法检测112例胆管癌组织和29例癌旁正常组织中AEG-1和VEGF的表达水平.结果 AEG-1和VEGF在胆管癌中的表达阳性率分别为71.4%和70.5%,高于癌旁正常组织0%(P<0.01).AEG-1在胆管癌中的表达阳性率与组织学分级(高分化45.5%、中分化64.9%、低分化86.8%)、临床分期(Ⅰ~Ⅱ期61.8%、Ⅲ~Ⅳa期91.7%)、淋巴结转移(无淋巴结转移61.8%,伴淋巴结转移91.7%)呈正相关(P<0.05),而与患者年龄和病理类型无明显相关(P>0.05).结论 胆管癌中AEG-1和VEGF高表达提示预后较差,可作为临床评估预后的指标.

Objective To investigate the expression and clinical significance of astrocyte elevated gene-1 (AEG-1) and vascular endothelial growth factor (VEGF) in cholangiocarcinoma.Methods The expression levels of AEG-1 and VEGF in bile duct neoplasms were detected by using immunohistochemistry,including 112 cases of malignant tumors and 29 cases of benign tissues.Results The positive expression rate of AEG-1 and VEGF was respectively 71.4% and 70.5% in malignant tumors,higher than 0% in benign tissues (P <0.01).The positive expression rate of AEG-1 was positively associated with histological classification (well-differentiated 45.5%,moderately differentiated 64.9%,and poorly differentiated 86.8%),clinical stage (stage Ⅰ-Ⅱ 61.8%,stage Ⅲ-Ⅳ a 91.7%) and lymph node metastasis (positive lymph node metastasis 61.8%,and negative lymph node metastasis 91.7%) (P < 0.05),but not with ages and pathologic types (P > 0.05).Conclusion Overexpression of AEG-1 and VEGF in cholangiocarcinoma suggests poor prognosis.They can be used as detection indexes to evaluate prognosis.
