目的 探讨腰椎关节突关节融合器固定的生物力学稳定性.方法 选用30具腰椎脊柱功能单元标本,分为对照组、融合器固定组、髓核半切组、髓核半切螺钉固定组和髓核半切融合器固定组,进行6个自由度的脊柱成角运动范围测量.结果 与对照组比较,融合器固定组在除后伸时的其他5个自由度的脊柱成角运动范围(ROM)差异有统计学意义(P<0.01).髓核半切术后,融合器固定组在6个自由度的ROM均小于髓核半切组(P<0.01),与螺钉固定组比较,在除前屈、后伸的4个自由度ROM差异无统计学意义(P>0.05).结论 关节突关节融合器植入后可以增加生理状态和髓核半切后脊柱功能单元的动态稳定性.

Objective To investigate biomechanical stability of lumbar zygapophysial joint instrumented with fusion cage.Methods Thirty functional spinal units were divided into control group,lumbar zygapophysial joint fusion cage fixation group,disc semi-resection group,disc semi-resection instrumented with facet screw fixation group and disc semi-resection instrumented with zygapophysial joint fusion cage fixation group.The pull-torsional biomechanical testing machine (MTS 858 System),optical positioning and motion capture system were used to test the angle range of motion (ROM) of functional spinal unit (FSU) under 6 degrees of freedom.Results There was significant difference in the angle ROM in other five degrees of freedom difference except extension orientation between lumbar zygapophysial joint fusion cage fixation group and control group (P < 0.01).After the nucleus hemi-section,the ROM of 6 degrees of freedom in fusion cage fixation group was obviously less than that in the nucleus hemi-section group (P < 0.01),while compared with the facet screw fixation group,there was no significant difference (P > 0.05) except in flexion and extension orientation.Conclusion Lumbar zygapophysial joint fusion cage implantation can strengthen the dynamic stability of functional spinal unit in physiological and nucleus hemi-section conditions.
