目的 探讨术前影像学检查判定肾脏小肿瘤(SRMs)性质的临床价值.方法 分析武汉协和医院收治59例SRMs患者临床资料,比较术前各种影像学检查在SRMs诊断中的优势及价值.结果 彩色多普勒超声、计算机断层扫描(CT)、磁共振成像(MRI)及正电子发射显像(PET)/CT判定SRMs良恶性的准确率分别为93.22%、94.73%、95.00%及100%;敏感性分别为97.87%、83.33%、94.12%及100%;特异性分别为75.00%、96.07%、100%及100%.结论 术前影像学检查有助于提示SRMs的性质,CT及MRI两者价值相当,具有重要参考价值,超声可作为筛查SRMs的首选方法,PET/CT诊断价值最高,但因价格高不作为常规检查.

Objective To explore the clinical values of preoperation image examinations for predicting properties of small renal masses (SRMs).Methods The clinical data of 59 patients with SRMs were retrospectively analyzed,and the advantage and value of preoperative different imaging examinations in the diagnosis of SRMs were compared.Results The accuracy rate of detecting malignant and benign of SRMs via ultrasound,computed tomography (CT),magnetic resonance imaging (MRI) and positron emission tomography (PET)/CT was 93.22%,94.73%,95.00% and 100%,respectively; the sensitivity was 97.87%,83.33%,94.12% and 100%,respectively,and the specificity was 75.00%,96.07%,100% and 100%,rspectively.Conclusion Preoperative imagings are conducive to predicate the nature of SRMs.CT and MRI have equivalent and crucial reference value.Ultrasonic can served as the preferred screening method for SRMs.PET/CT has the highest diagnostic value for SRMs,but not as a routine examination due to prohibitive costs.
