目的 通过两种手术入路消融术建立兔椎间盘退变模型,探讨其优缺点及可能性.方法 新西兰大白兔24只,随机分为腹膜外入路及侧方入路组各12只.分别通过侧方及腹膜外的手术入路暴露椎间盘,穿刺L3/4~ L5/6椎间隙并消融30 s.术前及术后行CT、MRI检查及病理学检查.结果 两组内CT检查术后各时间点差异均有统计学意义(P<0.01).4周两组间差异有统计学意义(P<0.01).MRI T2矢状位两组均可见髓核信号降低,两组信号差异不明显.病理学检查可见两组均可见髓核丢失及椎间盘纤维化.结论 两种手术入路均可建立与人相似的退变模型,侧方入路早期退变更为明显.

목적 통과량충수술입로소융술건립토추간반퇴변모형,탐토기우결점급가능성.방법 신서란대백토24지,수궤분위복막외입로급측방입로조각12지.분별통과측방급복막외적수술입로폭로추간반,천자L3/4~ L5/6추간극병소융30 s.술전급술후행CT、MRI검사급병이학검사.결과 량조내CT검사술후각시간점차이균유통계학의의(P<0.01).4주량조간차이유통계학의의(P<0.01).MRI T2시상위량조균가견수핵신호강저,량조신호차이불명현.병이학검사가견량조균가견수핵주실급추간반섬유화.결론 량충수술입로균가건립여인상사적퇴변모형,측방입로조기퇴변경위명현.
Objective To develop intervertebral disc (IVD) degeneration animal model by plasma radiofrequency ablation through two surgical approaches.Methods Twenty-four New Zealand white rabbitswere divided into Extreme-Lateral group and retroperitoneum group equally.L3/4-L5/6 IVDs were punctured and ablated for 30 s in two groups.CT imaging studies,MRI imaging studies and histological analysis were performed before operation,and 4,8,12 weeks after operation to monitor progression of IVD degeneration.Results The CT examinations showed there was significant difference in the degeneration signs at different time points between two groups (P < 0.01).The signals of nucleus pulposus were decreased in both two groups,and there was no significant difference between two groups.Pathologically,there was loss of nucleus pulposus and fibrosis of the IVDs in both two groups.Conclusion We built a practicable and reproducible animal IVD degeneration model via two possible surgical routes.The IVD degenerate fast at early time point via the far-lateral approach.
