目的 探讨雌雄大鼠脊髓损伤(SCI)后神经功能恢复的差异性和可能原因.方法 健康SPF级SD大鼠雌雄各30只,按性别分为两组:雄性大鼠组(A组)和雌性大鼠组(B组);改良Allen法在胸7节段平面制作SCI模型.于术后1、2、3、4、7d各组随机处死大鼠2只取损伤处脊髓检测超氧化物歧化酶(SOD)、丙二醇(MDA)和细胞凋亡,术后1d和1、2、3、4周采用改良大鼠胸段脊髓损伤后功能评判标准(BBB评分法)对大鼠进行行为学检查评分,同时各组随机处死1只大鼠取损伤处脊髓冰冻切片,苏木素-伊红(HE)染色显微镜下观察组织结构变化.结果 (1)BBB评分:脊髓损伤后2~4周时,两组大鼠的后肢功能均出现部分恢复,至第4周,B组评分:(8.250±1.658)分,A组为:(5.580±1.443)分,差异有统计学意义(P<0.01).(2)SOD活性变化:脊髓损伤2d时SOD活性最低,A组:(24.073 ±3.220) U/ml,B组:(42.663±3.940) U/ml;随后均逐渐升高,至7d时,A组SOD活性:(93.556 ±7.138) U/ml,B组:(106.833±5.878) U/ml,在1周内A组均低于B组(P<0.01).(3)MDA含量变化:脊髓损伤2d内两组MDA含量差异无统计学意义,至第3、4和7天时,A组MDA含量为(28.822±2.804)、(25.106±1.911)、(15.163±3.075) μmol/L;B组分别为:(24.224±2.627)、(18.377±2.416)、(10.959±2.780) μmol/L,差异有统计学意义(P<0.05).(4)细胞凋亡:在脊髓损伤3d时达高峰,A组:(187.83±18.30)个/视野,B组:(160.67±8.34)个/视野;至7d时,A组:(84.00±10.96)个/视野,B组:(56.00±7.43)个/视野;在3、4、7d时A组细胞凋亡数明显高于B组(P<0.01).(5)病理切片观察,损伤脊髓组织的炎症浸润、空洞形成、白质留存、瘢痕形成等方面差异有统计学意义(P<0.05),B组优于A组.结论 雌性大鼠SCI后运动功能的恢复优于雄性大鼠;其原因可能是雌性大鼠体内相对高水平的雌激素存在,通过抗氧化作用和抑制细胞凋亡,减轻了继发性损伤.

Objective To explore the difference of the neural function and some reasons in male and female rats after spinal cord injury (SCI).Methods By the striking of the spinal cord at the level of T7 with Allen's device,acute SCI was induced in anesthetized rats.Sixty SCI model rats were divided into 2 groups by gender:group A (male group) and group B (female group),each with 30 SD rats.The changes of superoxide dismutase (SOD),malondialdehyde (MDA) and cell apotosis in the lesion site were observed in 1,2,3,4 and 7 days after SCI.The motor functional changes were evaluated by BBB scale in 1 day and 1,2,3,4 weeks after SCI.And at the same time point,the injuried spinal cord was made into frozen section,histomorphology change was observed by hematoxylin and eosin (HE) staining.Results The motor capacity of hind limb was some obvious recovery in every group in 2-4 w after modeling.The BBB scores were significantly higher in group B (8.25 ±1.658) than in group A (5.58 ±1.443) after 4 w (P<0.01).The SOD activity was the minimum in both group A [(28.822±2.804) U/ml] and group B [(106.833 ±5.878) U/ml] in 2 d.To 7 d,it were (93.556 ±7.138) U/ml in group A and (106.833 ±5.878) U/ml in group.It was higher in group B than group A in 7 d (P < 0.01).The evel of MDA was no difference in 2 d after SCI modeling.To 3,4 and 7 d,it was (28.822 ±2.804),(25.106 ± 1.911),(15.163 ±3.075) μnol/L in group A,and (24.224 ± 2.627),(18.377 ± 2.416),(10.959 ± 2.780) μmol/L in group A (P < 0.05).The biggest number of apoptotic cells was (187.83 ± 18.30) cell/view in group A and (160.67 ± 8.34) cell/view in group B after 3 d.It was (84.00 ± 10.96) cell/view in group A and (56.00 ± 7.43) cell/view in group B in 7 d.It was less in goup B than in group A in 2,3,4,7 d (P < 0.01).Group B was better than group A in pathological changes of spinal cord injury,incloud inflammatory,cavitation,white matter retained,scar formation.Condusion The recovery of motor capacity of hind limb is better after SCI in female rats than in male rats; the reason may be that there are more estrogen in female rat than in male rat,because estrogens can reduce oxygen free radicals and adip-peroxidation,decrease apoptosis of nerve cell in the injury site of spinal cord.
