目的 利用三维有限元评价前路特殊塑形钛板加方形区螺钉联合后柱拉力螺钉内固定治疗累及方形区的复杂髋臼骨折的生物力学稳定性.方法 按Letournel分型,建立“T”形(Ⅰ)、前柱+后半横(Ⅱ)及双柱骨折(Ⅲ)模型.再分别模拟双柱钛板内固定(A)、单纯前路特殊塑形钛板加方形区螺钉内固定(B)及前路特殊塑形钛板加方形区螺钉联合后柱拉力螺钉内固定(C)治疗此3种骨折并模拟站立位时,在正常载荷下对3种内固定方式进行生物力学稳定性比较.结果 在各种骨折模型上,3种内固定与正常模型比较,骨折线上各节点的平均位移为C<B<正常模型<A,差异无统计学意义(P>0.05).结论 前路特殊塑形钛板加方形区螺钉联合后柱拉力螺钉内固定治疗累及方形区的复杂髋臼骨折具有良好生物力学稳定性.

Objective To evaluate biomechanical properties of anterior reconstruction plate and trans-plate quadrilateral screws combinated with lag screw internal fixation in the posterior column of acetabular by three-dimensional finite element for providing theory basis to choice the internal fixation ways for complex acetabular fractures with fracture of quadrilateral.Methods According to Letoumel classification,T-shaped (Ⅰ),anterior column plus posterior hemitransverse (Ⅱ) and both-column (Ⅲ) acetabular fracture model was established.The biomechanics of double-column plate (A),anterior reconstruction plate and trans-plate quadrilateral screws (B) and anterior reconstruction plate and trans-plate quadrilateral screws combinated with lag screw itermal fixation in the posterior column of acetabular (C) on the standing position and normal load were analyzed,when each acetabular fracture model was internal fixation by them.Results The mean displacement of notes along fracture line on each fracture model is C < B < normal model < A,while no significant difference each other (P > 0.05).Conclusion Complex acetabular fractures with fracture of quadrilateral was internal fixed with anterior reconstruction plate and trans-plate quadrilateral screws combinated with lag screw internal fixation in the posterior column of acetabular has a good biomechanical stability.
