目的 探讨吲哚胺-2,3双加氧酶(IDO)在脓毒症患者免疫炎症进展过程中的表达及其临床意义.方法 选取90例脓毒症患者,按照疾病严重程度分为脓毒症组(32例)、严重脓毒症组(30例)及脓毒性休克组(28例);选择30例健康人作为对照组.采用流式细胞仪检测CD4+、CD8+、CD4+ CD25+调节性T细胞(Treg)的水平,计算CD4 +/CD8+百分比.采用酶联免疫吸附试验(ELISA)检测白细胞介素(IL)-6、IL-10、干扰素-γ(INF-γ)、IDO的表达水平.结果 对照组、脓毒症组、严重脓毒症组、脓毒性休克组IL-6水平分别为(45.23±8.0)、(86.84±7.83)、(142.01±11.11)、(206.92±17.79) ng/L,各组间比较差异有统计学意义(P<0.05);IL-10水平分别为(9.34±3.58)、(20.17±4.04)、(39.04±4.29)、(64.96±4.41)n#L,各组间比较差异有统计学意义(P<0.05);IL-6/IL-10分别为6.45±1.17、4.53±1.03、3.86±0.83、3.20 ±0.62,各组间比较差异有统计学意义(P <0.05);INF-y分别为(2.58±1.36)、(5.68±0.59)、(17.29±2.46)、(31.28±4.49) ng/L,各组间比较差异有统计学意义(P<0.05);IDO分别为(13.41±1.80)、(22.88±3.61)、(34.38±2.2)、(45.35±4.29) ng/L,各组间比较差异有统计学意义(P<0.05);CD4+分别为(37.35±1.79)%、(29.38±1.53)%、(26.80±0.97)%、(21.22±1.44)%,各组间比较差异有统计学意义(P<0.05);CD8+分别为(19.61±0.73)%、(20.33±1.01)%、(23.1±0.98)%、(25.63±0.52)%,各组间比较差异有统计学意义(P<0.05);CD4 +/CD8+分别为1.91±0.15、1.42±0.07、1.16±0.08、0.83±0.07,各组间比较差异有统计学意义(P<0.05);Treg分别为(0.62 ±0.11)%、(1.96±0.87)%、(3.12±1.02)%、(4.35±1.23)%,各组间比较差异有统计学意义(P<0.05).结论 IDO在免疫炎性反应中发挥重要作用,有助于评估脓毒症的严重程度及预后.

Objective To investigate the expression of indoleamine 2,3-dioxygenase (IDO) in patients with sepsis during the immune inflammation progression,and the role of IDO in immune inflammation in sepsis.Methods A total of 90 cases of sepsis were collected from June 2011 to October 2013 during their stay in our hospital,and divided into three groups according to their disease severity:sepsis group (n =32),severe sepsis group (n =30) and septic shock group (n =28).Thirty healthy individuals served as control group.The percentages of CD4 +,CD8 +,and CD4 + CD25 + regulatory T cells (Tregs)were detected by flow cytometry.The CD4+/CD8 + ratio was calculated.Interleukin (IL)-6,IL-10,INF-γ,and IDO concentrations were detected by enzyme-linked immunosorbent assay (ELISA).Results With the increased inflammatory response,IL-6,IL-10,INF-γ,and IDO concentrations were gradually increased,and the percentate of CD8 + Tregs was gradually increased with the difference being statistically significant among all groups (P < 0.01 for all).IL-6 level was decreased,and concentrations of IL-10 and CD4 + and the CD4 +/CD8 + ratio were also gradually reduced with the difference being statistically significant among all groups (P < 0.01 for all).Conclusion IDO may play an important role in immune inflammatory response,which may help to assess the severity and prognosis of sepsis.
