目的探讨锁定加压钢板(LCP)外固定治疗股骨远端骨折术后感染的临床疗效.方法 2007年2月至2011年12月间,采用LCP外固定治疗股骨远端骨折术后感染患者6例,其中男4例,女2例;年龄24 ～50岁,平均年龄36.5岁;骨折AO分型:A1型4例,A2型2例;开放性骨折3例;Gustilo分型:Ⅰ型2例,Ⅱ型1例;闭合性骨折3例.结果 6例患者均获得随访,患者均获得骨愈合,平均愈合时间为20.6周,术后6个月膝关节平均屈曲110.7°,术后1年膝关节疼痛与关节活动度进行评估(HSS评分),平均90.5分.结论 LCP外固定治疗股骨远端骨折术后感染具有感染控制彻底、骨折固定稳定及膝关节功能恢复良好等特点.

Objective To evaluate the clinical effects of postoperative infection in the distal femoral fractures with external fixation of locking compression plate.Methods From February 2007 to December 2011,6 cases of infected distal femoral fractures were healed with external fixation of locking compression plate.The mean age of patients was 36.5 years (24-50 years),and there were 4 males and 2 females.According to the AO classification,there were 4 cases of type A1 and 2 of type A2.There were 3 cases of open fracture,including 2 cases of type Ⅰ and 1 of type Ⅱ,according to the Gustilo classification,and 3 cases of closed fractures.Results All 6 patients were available at followed up.All fractures were treated for union.The healing time of fractures was 20.6 weeks averagely.The flexion of knee achieved 110.7° at the 6th month.According to the knee HSS system,the score was 90.5 points averagely one year after operation.Conclusion The technique of locking compression plate for the treatment of infected fractures of the distal femoral fractures has many advantages,such as controlled infection completely,stable fixation and maximal recovery of the knee function.
