目的 检测喉鳞状细胞癌患者外周血及癌组织中Th9细胞及白细胞介素(IL)-9的表达水平,探讨其与喉鳞状细胞癌患者临床病理特征的关系.方法 选择66例初治的喉鳞状细胞癌患者和40例健康志愿者.采用流式细胞仪检测外周血Th9细胞水平,酶联免疫吸附法(ELISA)检测血清中IL-9表达水平.应用免疫组织化学染色分析癌旁组织及癌组织中IL-9阳性细胞的浸润程度.分析喉鳞状细胞癌患者外周血Th9细胞、血清中IL-9水平及癌组织中IL-9阳性细胞浸润程度与各临床病理参数间的关系.结果 喉鳞状细胞癌患者外周血中Th9细胞及IL-9水平分别为(2.64±0.24)%和(89.40 ±6.66) ng/L,显著高于健康对照组[(1.07±0.11)%,t=12.63,P<0.01;(26.80±3.76) ng/L,t=14.37,P<0.01],Ⅲ~Ⅳ期患者的Th9细胞比例及IL-9水平明显高于Ⅰ~Ⅱ期患者[(3.21±0.22)%比(2.07±0.19)%,t=8.24,P<0.01;(104.30±6.07) ng/L比(72.90±5.37) ng/L,t=8.91,P<0.01].喉鳞状细胞癌患者癌组织中IL-9阳性细胞浸润率为84.37%(27/32),而对应癌旁组织为21.87%(7/32),差异有统计学意义(x2=25.098,P<0.01).外周血Th9细胞和IL-9的表达水平与喉鳞状细胞癌的临床分期、肿瘤分化程度、区域淋巴结转移明显相关(P<0.05),与年龄、性别、原发灶部位无相关(P>0.05).癌组织中IL-9阳性细胞浸润程度与临床分期、区域淋巴结转移明显相关(P<0.05),与其他临床病理特征无相关(P>0.05).结论 喉鳞状细胞癌患者外周血Th9细胞比例及血浆中IL-9水平显著升高,且与临床分期等病理指标明显相关,提示Th9细胞及IL-9参与喉鳞状细胞癌的发生发展.

Objective To detect the expression of T helper cells 9 (Th9) in the peripheral blood,interleukin (IL)-9 level in the serum and the IL-9 in the tumor tissues of patients with laryngeal squamous cell carcinoma (LSCC),and analyze the relationship between Th9/IL-9 and clinical characteristics and their possible roles in the immunological pathogenesis of LSCC.Methods Sixty-six patients with LSCC and 40 healthy controls were included.Th9 cells in the peripheral blood were detected by flow cytometry.The serum IL-9 level was measured by enzyme-linked immunosorbent assay (ELISA).Immunohistochemistry (IHC) was used to determine the IL-9 expression in paraffin-embedded tissues of patients with LSCC.The relationship between Th9/IL-9 and clinical characteristics was analyzed.Results The level of Th9 cells in peripheral blood and IL-9 levels in serum of patients with LSCC were (2.64 ± 0.24) % and (89.40 ± 6.66) ng/L,respectively,significantly higher than those of healthy controls [(1.07 ± 0.11)%,t =12.63,P <0.01 ; (26.80 ±3.76) ng/L,t =14.37,P <0.01].Patients with Ⅲ-Ⅳ stage had higher Th9 cells and IL-9 levels than those with Ⅰ-Ⅱ stage [(3.21 ± 0.22) % vs.(2.07 ± 0.19) %,t =8.24,P <0.01 ; (104.30 ±6.07) ng/L vs.(72.90 ±5.37) ng/L,t =8.91,P <0.01].Compared with peritumor tissues,intratumor tissues showed higher expression of IL-9 + cells (84.37% vs.21.87%,x2 =25.098,P < 0.01).The levels of Th9 cells in peripheral blood and IL-9 in serum were associated with tumor stage,tumor differentiation and regional lymph node metastasis (P < 0.05),while the IL-9 + cells in tumor tissues were associated with tumor stage and regional lymph node metastasis (P < 0.05).Conclusion There are higher quantification of Th9 cells and IL-9 in the peripheral blood and intratumor tissues in patients with LSCC,and they have positive correlation with the clinical stage.Th9 cells and IL-9 may contribute to the disease progression and prognosis of LSCC.
