目的 观察外源性肝再生增强因子(ALR)对肝硬化大鼠50%肝切除术后余肝再生能力的影响.方法 基因重组技术构建携带人ALR基因片段的慢病毒表达载体,转染大鼠正常肝细胞BLR 3A,验证其有效性与安全性;42只前期研究构建的肝硬化大鼠模型行50%肝切除,同时给予门静脉注射携带人ALR慢病毒表达载体,分别于术前、术后12、24、72 h、7、14、28 d检测各组大鼠余肝组织ALR mRNA与蛋白的表达、肝再生率及门静脉血液中丙氨酸转氨酶(ALT)与天冬氨酸转氨酶(AST)含量,并与前期研究结果行对比分析.结果 成功构建携带人ALR慢病毒表达载体,转染大鼠肝细胞BLR 3A,转染率为81.29%,可使细胞稳定表达ALR基因;构建的慢病毒载体可使大鼠模型余肝组织稳定表达ALR基因;ALR转染组大鼠术后72 h~28 d肝再生率分别为(51.14±6.63)%、(78.28±1.70)%、(81.71±3.24)%及(95.72±2.55)%,均高于空白对照组(P<0.05),且术后72 h及7d肝再生率高于血管内皮生长因子(VEGF)165转染组(P<0.05);术后72 h至28 d大鼠血清ALT含量分别(255.32 ±6.80)、(208.96±4.82)、(168.27±13.16)、(122.20±12.36) U/L,血清AST含量分别为(548.29±11.91)、(489.04±10.52)、(414.17 ±5.80)、(365.33±10.62) U/L均低于空白对照组(P<0.05),术后各时点血清ALT及AST含量与VEGF165转染组比较差异无统计学意义(P>0.05).结论 外源性人ALR可促进肝硬化大鼠肝部分切除术后余肝的再生,且在促进肝脏质量恢复方面优于外源性VEGF165.

목적 관찰외원성간재생증강인자(ALR)대간경화대서50%간절제술후여간재생능력적영향.방법 기인중조기술구건휴대인ALR기인편단적만병독표체재체,전염대서정상간세포BLR 3A,험증기유효성여안전성;42지전기연구구건적간경화대서모형행50%간절제,동시급여문정맥주사휴대인ALR만병독표체재체,분별우술전、술후12、24、72 h、7、14、28 d검측각조대서여간조직ALR mRNA여단백적표체、간재생솔급문정맥혈액중병안산전안매(ALT)여천동안산전안매(AST)함량,병여전기연구결과행대비분석.결과 성공구건휴대인ALR만병독표체재체,전염대서간세포BLR 3A,전염솔위81.29%,가사세포은정표체ALR기인;구건적만병독재체가사대서모형여간조직은정표체ALR기인;ALR전염조대서술후72 h~28 d간재생솔분별위(51.14±6.63)%、(78.28±1.70)%、(81.71±3.24)%급(95.72±2.55)%,균고우공백대조조(P<0.05),차술후72 h급7d간재생솔고우혈관내피생장인자(VEGF)165전염조(P<0.05);술후72 h지28 d대서혈청ALT함량분별(255.32 ±6.80)、(208.96±4.82)、(168.27±13.16)、(122.20±12.36) U/L,혈청AST함량분별위(548.29±11.91)、(489.04±10.52)、(414.17 ±5.80)、(365.33±10.62) U/L균저우공백대조조(P<0.05),술후각시점혈청ALT급AST함량여VEGF165전염조비교차이무통계학의의(P>0.05).결론 외원성인ALR가촉진간경화대서간부분절제술후여간적재생,차재촉진간장질량회복방면우우외원성VEGF165.
Objective To study the effects of exogenous augmenter of liver regeneration (ALR) on the capacity of liver regeneration in hepatocirrhosis after partial hepatectomy in rats.Methods The lentivirus vector containing human ALR gene was constructed by gene recombinant technology.The BLR 3A rat liver cells were transfected by lentivirus vector containing human ALR gene,then the transfect efficiency and security were tested.The lentivirus vector containing human ALR gene was injected through the portal vein of 42 cirrhotic rat models after hemihepatectomy.As compared with previous study results,the expression of ALR mRNA and protein in liver tissue,liver regeneration rate,the positive rate of proliferating cell nuclear antigen (PCNA),alanine transaminase (ALT) and aspartate aminotransferase (AST) levels in the blood of portal vein were detected before and 12 h,24 h,72 h,7 d,14 d,and 28 d after operation.Results The lentivirus vector containing human ALR was successfully constructed.The transfection efficiency was 81.29% in the BLR 3A rat liver cells after transfection with the lentivirus vector.The lentivirus vector made the BLR 3A rat liver cells express ALR gene steadily.The hepatic tissue also could express ALR gene stably.The liver regeneration rate in ALR group [(51.14±6.63)%,(78.28± 1.70)%,(81.71 ±3.24)% and (95.72 ± 2.55) %] during 72 h to 28 d after operation was obviously higher than that in the control group (P < 0.05),and the liver regeneration rate at 72 h and 7 d was also higher than that in vascular endothelial growth factor (VEGF) 165 group (P < 0.05).The serum levels of ALT [(255.32 ± 6.80),(208.96 ± 4.82),(168.27 ± 13.16) and (122.20 ± 12.36) U/L)] and AST [(548.29 ± 11.91),(489.04 ± 10.52),(414.17 ±5.80) and (365.33 ± 10.62) U/L] during 72 h to 28 d after operation in ALR group were evidently lower than those in the control group (P < 0.05),but there was no significant difference between ALR group and VEGF165 group.Conclusion The exogenous ALR has significantly positive effect on liver regeneration after partial hepatectomy in rats with hepatocirrhosis,but the exogenous ALR was better then VEGF165 in terms of the recovery of the liver quality.
