目的 观察博莱霉素涂层支架对兔胆管良性狭窄的影响.方法 24只新西兰白兔行胆管损伤后平均分入A、B组,分别将博莱霉素涂层支架和普通支架植入两组兔的肝外胆管内.0d、4周后,取耳缘静脉血,检测总胆红素变化,随后处死兔,取植入支架行石蜡切片,行苏木素-伊红(HE)染色,并采用计算机图像检测系统检测两组胆管腔面积变化,同时各组切片行Fas、转化生长因子-β1(TGF-β1)、α-平滑肌肌动蛋白(α-SMA)抗体进行免疫标记,比较两组变化.结果 A组兔胆管组织中Fas基因表达水平明显高于B组(P<0.05):两组比较,TGF-β1、α-SMA的表达水平前者低于后者(P<0.05):A组肝外胆管管腔狭窄减轻程度及肝功能改善程度较B组明显(P<0.05).结论 博莱霉素通过增强兔平滑肌细胞Fas基因表达,促进凋亡,抑制成纤维细胞活性,减少胶原沉积,从而抑制兔肝外胆管狭窄,促进肝功能恢复.
目的 觀察博萊黴素塗層支架對兔膽管良性狹窄的影響.方法 24隻新西蘭白兔行膽管損傷後平均分入A、B組,分彆將博萊黴素塗層支架和普通支架植入兩組兔的肝外膽管內.0d、4週後,取耳緣靜脈血,檢測總膽紅素變化,隨後處死兔,取植入支架行石蠟切片,行囌木素-伊紅(HE)染色,併採用計算機圖像檢測繫統檢測兩組膽管腔麵積變化,同時各組切片行Fas、轉化生長因子-β1(TGF-β1)、α-平滑肌肌動蛋白(α-SMA)抗體進行免疫標記,比較兩組變化.結果 A組兔膽管組織中Fas基因錶達水平明顯高于B組(P<0.05):兩組比較,TGF-β1、α-SMA的錶達水平前者低于後者(P<0.05):A組肝外膽管管腔狹窄減輕程度及肝功能改善程度較B組明顯(P<0.05).結論 博萊黴素通過增彊兔平滑肌細胞Fas基因錶達,促進凋亡,抑製成纖維細胞活性,減少膠原沉積,從而抑製兔肝外膽管狹窄,促進肝功能恢複.
목적 관찰박래매소도층지가대토담관량성협착적영향.방법 24지신서란백토행담관손상후평균분입A、B조,분별장박래매소도층지가화보통지가식입량조토적간외담관내.0d、4주후,취이연정맥혈,검측총담홍소변화,수후처사토,취식입지가행석사절편,행소목소-이홍(HE)염색,병채용계산궤도상검측계통검측량조담관강면적변화,동시각조절편행Fas、전화생장인자-β1(TGF-β1)、α-평활기기동단백(α-SMA)항체진행면역표기,비교량조변화.결과 A조토담관조직중Fas기인표체수평명현고우B조(P<0.05):량조비교,TGF-β1、α-SMA적표체수평전자저우후자(P<0.05):A조간외담관관강협착감경정도급간공능개선정도교B조명현(P<0.05).결론 박래매소통과증강토평활기세포Fas기인표체,촉진조망,억제성섬유세포활성,감소효원침적,종이억제토간외담관협착,촉진간공능회복.
Objective To investigate the effects of bleomycin eluting stent on benign biliary stricture.Methods Twenty-four New Zeal white rabbits with bile duct injury were randomly assigned into group A (n =12) and group B (n =12),subject to the implantation of the extrahepatic bile duct bleomycin-coated stents and ordinary stents.After 0 d and 4 weeks,the ear vein blood was taken for determination of total bilirubin,then the rabbits were sacrifaced,the papaffin-embedded sections were prepared by the implanted stents,and stained with hematoxylin and eosin (HE).The computer image detection system was used to examine the changes of gallbladder lumen areas in two groups.Fas,transforming growth factor-β1 (TGF-β1),and α-smooth muscle actin (α-SMA) antibodies were employed for immunostaining in two groups.Results Fas gene expression level in bile duct tissue of group A was significantly higher than in group B (P < 0.05).TGF-β1 and α-SMA expression levels in group A were significantly lower than in group B (P < 0.05).The extrahepatic bile duct stenosis was significantly alleviated,and liver function improved significantly in group A as compared with group B (P < 0.05).Conclusion Bleomycin enhances the expression of Fas gene in rabbit smooth muscle cells,promotes apoptosis,inhibits fibroblast activity,and decreases the deposit of collagen,resulting in the suppression of bile duct stricture,and the recovery of liver function.
