目的评价全覆膜金属胆道支架在治疗良性胆管狭窄中的应用价值.方法回顾性总结53例行内镜下置入全覆膜金属胆道支架治疗的良性胆管狭窄患者的临床资料,分析支架置入和取出情况、疗效和并发症发生情况.结果 53例均一次性成功置入全覆膜金属胆道支架.支架放置时间2～186 d,平均放置时间138 d.支架取出后随访6个月,胆管狭窄均得到缓解,短期有效率为100.0％;继续随访12个月,6例再发胆管狭窄,其余47例胆管狭窄长期缓解,长期有效率为88.7％(47/53).出现术后胰腺炎1例、支架移位3例、胆管炎5例、高淀粉酶血症9例、胆泥33例,无一例出血、穿孔发生.结论全覆膜金属胆道支架治疗良性胆管狭窄是安全、有效的,大多数患者临床症状可以得到长期缓解.

Objective To evaluate the clinical effectiveness of fully covered metal stent(FCMT) for patients with benign biliary stricture(BBS).Methods The clinical data of 53 patients with benign biliary strictures who were treated with placement of FCMT were retrospectively analyzed.Results All FCMT were successfully placed in 53 patients and removed after a mean time of stent placement of 138 d(ranging 2-186 d).Biliary stricture was resolved in all patients upon the follow-up at 6 months after stent removal with the efficiency rate being 100％.Upon follow-up at 12 months after stent removal,biliary stricture relapsed in 6 patients,achieving a stricture resolution rate of 47/53 (88.7％).Postoperative complication included pancreatitis (n =1),stent migration (n =3),cholangitis (n =5),hyperamylasemia(n =9) and biliary sludge (n =33).No hemorrhage or perforation was observed.Conclusion The use of FCMS for BBS is safe and leads to long-term clinical resolution.
