目的 探讨尿沉渣与干化学联合检测在泌尿系统感染中的意义.方法 采用尿沉渣分析仪和尿干化学分析仪对2011年1-12月青岛大学医院第二附属医院526份尿培养标本进行检测,以细菌培养结果为金标准,判断尿白细胞、细菌计数及亚硝酸盐三者单项分析和联合分析在泌尿系统感染中的敏感性和特异性.结果 在单项指标分析中,细菌计数的敏感度、特异度、阳性预测值、阴性预测值、与细菌培养符合率最高,分别为84.3%、90.5%、84.3%、90.5%、88.2%;在两项指标联合分析中,细菌计数和白细胞计数联合分析的敏感度、特异度、阳性预测值、与细菌培养符合率最高,分别为83.9%、91.0%、85.4%、88.2%,而白细胞计数和亚硝酸盐联合分析的阴性预测值最高,为91.3%.当三个指标联合分析时,与两两联合、单项分析相比较,其敏感度、特异度、阳性预测值、阴性预测值、与细菌培养符合率最高,分别为85.4%、92.6%、84.7%、93.0%、90.3%.结论 综合分析尿沉渣分析仪和尿干化学分析仪检测的尿液指标可以作为泌尿系统感染的一项快速筛检指标.
目的 探討尿沉渣與榦化學聯閤檢測在泌尿繫統感染中的意義.方法 採用尿沉渣分析儀和尿榦化學分析儀對2011年1-12月青島大學醫院第二附屬醫院526份尿培養標本進行檢測,以細菌培養結果為金標準,判斷尿白細胞、細菌計數及亞硝痠鹽三者單項分析和聯閤分析在泌尿繫統感染中的敏感性和特異性.結果 在單項指標分析中,細菌計數的敏感度、特異度、暘性預測值、陰性預測值、與細菌培養符閤率最高,分彆為84.3%、90.5%、84.3%、90.5%、88.2%;在兩項指標聯閤分析中,細菌計數和白細胞計數聯閤分析的敏感度、特異度、暘性預測值、與細菌培養符閤率最高,分彆為83.9%、91.0%、85.4%、88.2%,而白細胞計數和亞硝痠鹽聯閤分析的陰性預測值最高,為91.3%.噹三箇指標聯閤分析時,與兩兩聯閤、單項分析相比較,其敏感度、特異度、暘性預測值、陰性預測值、與細菌培養符閤率最高,分彆為85.4%、92.6%、84.7%、93.0%、90.3%.結論 綜閤分析尿沉渣分析儀和尿榦化學分析儀檢測的尿液指標可以作為泌尿繫統感染的一項快速篩檢指標.

Objective To assess the diagnostic values of urinalysis in urinary tract infections.Methods A total of 526 urine culture specimens were analyzed by UF-1000i urine analyzers and GEB600 urine chemistry analyzer to determine the sensitivity and specificity of individual and combined analysis of white blood cell,bacteria count and nitrite test in the diagnosis of urinary tract infections.And bacterial culture was used as the gold standard.Results The sensitivity,specificity,positive predictive value,negative predictive value and accuracy rate of bacterial count were the highest in individual analysis at 84.3%,90.5%,84.3%,90.5% and 88.2% respectively.The sensitivity,specificity,positive predictive value and accuracy of combined bacterial count and leukocyte was 83.9%,91.0%,85.4% and 88.2% respectively while the negative predictive value of white blood cell and nitrite was highest (91.3%).Compared with the combined or single analysis,the combination of three analyses had the highest sensitivity,specificity,positive predictive value,negative predictive value and accuracy rate at 85.4%,92.6%,84.7%,93.0%,90.3% respectively.Conclusion The combined urinalysis may be used as a rapid screening indicator of urinary tract infections.
