目的探讨偏颌畸形的治疗方法及效果.方法对19例偏颌畸形患者根据畸形部位和程度,分别采用LefortⅠ型截骨、下颌升支矢状劈开截骨、颏部水平截骨、下颌前牙根尖下截骨和假体置入等手术方式,并观察其临床疗效.结果术后随访19例,平均随访12个月,患者面形、(牙合)关系有较大改善,效果满意,所有截骨线愈合良好,无牙根损伤、神经损伤等并发症发生.结论根据偏颌畸形类型及程度选择手术方式,配合术前、术后正畸治疗可得到满意效果,下颌前牙根尖下截骨结合下颌骨整形术治疗轻度的偏颌畸形效果满意.

Objective To explore the treatment of laterognathism of the mandible.Methods Nineteen cases of laterognathism of the mandible were treated by Lefort Ⅰ osteotomy,sagittal split ramus osteotomy,genioplasty,anterior mandibular subapical osteotomy and prosthesis implantation,respectively,and their efficacy was evaluated and clinical experience summarized.Results The nineteen cases were followed up for average 12 months after the treatments and all the patients were satisfied with the surgical effects.All osteotomy lines healed well and there were no complications,such as root damage or neurotrosis.Conclusions The orthognathic procedure for laterognathism of the mandible should be selected according to the types and locations of asymmetry deformities.Anterior mandibular subapical osteotomy combined with mandibular cosmetic surgery for slight laterognathism of the mandible could achieve satisfactory results.
