目的探讨带蒂旋髂浅动脉皮瓣在整形烧伤外科的临床应用.方法 19例皮肤软组织缺损的病例,包括手腕及前臂、下腹部及会阴、大腿根部等处各种创面,均应用带蒂旋髂浅动脉皮瓣修复.19例中15例经过创面彻底扩创,3例瘢痕患者切除瘢痕后,均应用带蒂旋髂浅动脉皮瓣转移修复,皮瓣最小3.0cm×4.5 cm,最大26.0 cm ×22.0 cm,蒂长5～7 cm.结果本组18例皮瓣均一期成活,1例皮瓣远端出现少许坏死,再次扩创后完全修复创面.术后随访3个月至1年半,移植皮辨质地柔软,功能及形态良好.结论带蒂旋髂浅动脉皮瓣切取方便、血运丰富、质地良好、易于成活、修复范围大,是修复手、前臂、下腹、会阴部等处软组织缺损的较好方法.
목적 탐토대체선가천동맥피판재정형소상외과적림상응용.방법 19례피부연조직결손적병례,포괄수완급전비、하복부급회음、대퇴근부등처각충창면,균응용대체선가천동맥피판수복.19례중15례경과창면철저확창,3례반흔환자절제반흔후,균응용대체선가천동맥피판전이수복,피판최소3.0cm×4.5 cm,최대26.0 cm ×22.0 cm,체장5～7 cm.결과 본조18례피판균일기성활,1례피판원단출현소허배사,재차확창후완전수복창면.술후수방3개월지1년반,이식피변질지유연,공능급형태량호.결론 대체선가천동맥피판절취방편、혈운봉부、질지량호、역우성활、수복범위대,시수복수、전비、하복、회음부등처연조직결손적교호방법.
Objective To investigate the clinical application of pedicled superficial circumflex iliac artery (SCIA) flaps in Burns and Plastic Surgery.Methods 19 cases with skin and soft tissue defects were repaired with the Superficial circumflex iliac artery flaps.The defects located in the hand and forearm,the lower abdomen and perineal area,the radicle area of thigh,etc.Before the SCIA flaps were transfered to the recipient areas,15 cases with different kinds of wounds were debrided thoroughly,3 cases with scar were removed directly.The size of the flaps ranged from 3.0 cm × 4.5 cm to 26.0 cm × 22.0 cm,and the pedical was 5 cm to 7 cm in length.Results The flaps in the 18 cases survived completely.Skin necrosis in the distal end of the flap appeared in 1 case,and the wound healed after the second repair.The follow-up period rang from 3 to 18 months.The apperance and function of the hand or foot was satisfactory.Conclusions Because the pedicled SCIA flaps can be obtain conveniently and contain sufficient bloodsupply,so the flap is easy to survive and the flap can be designed in a large size.We believe it is an ideal method to use this flap to repair skin and soft tissue defects located in hands,forearms,the lower abdomen and perineal areas,the radicle area of thigh and so on.
