目的探讨经皮微创椎弓根钉植入治疗单节段胸腰段压缩骨折的可行性.方法将40例新鲜单节段胸腰椎骨折患者随机分为两组,经皮微创组16例,采用经皮微创椎弓根钉经伤椎的上下相邻椎体撑开复位固定,利用C型臂X线透视机定位弓根钉进钉点并经皮小切口旋入椎弓根钉,然后在髂嵴肌深层潜行安装尾杆固定.传统开放手术组24例,采用传统开放手术治疗.比较两组的疗效.结果两组患者术后伤椎前缘高度百分比、后凸Cobb's角等较术前均有显著改善(P＜0.01),两组间术前、术后影像学观察指标比较差异无统计学意义(P＞0.05),但经皮微创较传统开放组的手术时间、住院时间和带支具离床时间明显缩短,术中出血量和术后引流量明显减少.结论经皮椎弓根螺钉微创技术具有创伤小、出血少、术后恢复快等优点,是治疗单节段无神经症状胸腰椎骨折的有效方法.

Objective To evaluate the feasibility of percutaneous transpedicular screw fixationtechniques in the treatment of single thoracolumbar vertebra fractures.Methods Forty patients with single segment fractures of thoracic and lumbar vertebrae were treated; sixteen cases undwent percutaneous operation and twenty-four cases underwent open operation.Under general anesthesia,traction reduction was performed,and then under the guidance of C-arm X-ray,the fixation was carried out into the deep iliac muscles.Results There were significant increase in postoperative vertebral heisht percent and Cobb' s angle.There were significant difference in operative time,length of stay in bed and the volume of blooding between percutaneous operation group and open operation group.Conclusions The technique of minimally invasive percutaneous operation to treat single segment of thoracolumbar fractures,which has some advantages including fewer traumas,less blood lose and better recovery from operation.
