目的 探讨超声引导下麦默通微创旋切系统(MMT)在乳腺微创外科围术期的应用价值.方法 在高频及三维超声引导下对386例乳腺肿块患者的546处乳腺病灶应用MMT微创切除和(或)活检术,分析在乳腺微创外科中的应用价值.结果 术中超声检查证实肿块完整切除.病理结果显示良性病变378例538处肿块,其中乳腺纤维瘤372处肿块,乳腺腺病148处肿块,乳腺导管内乳头状瘤18处肿块.恶性病变8例8处肿块,其中非浸润性导管癌3例,小叶原位癌5例.术后临床和超声检查定期随访均未发现肿块复发.结论 超声引导下麦默通旋切系统对3 cm以下乳腺病灶可进行微创完整切除并可获得明确的病理学诊断,具有定位准确、安全可靠、穿刺创伤小、并发症少等优点.

Objective To investigate the value of ultrasound-guided mammotome system on the perioperative period of minimally invasive breast surgeries.Methods Under high-frequency and three-dimensional ultrasound-guided mammotome systeam,546 breast lesions of 386 cases were removed and (or) biopsied to analyze the value of minimally invasive breast surgeries.Results Tumors were completely removed in all.Pathologic results showed that 538 breast lesions of 378 cases of benign lesions,including 372 lesions in breast fibroma,148 lesions in breast adenosis,18 lesions in breast intraductal papilloma.Eight breast malignant lesions of 8 cases,including 3 cases of non-invasive ductal carcinoma,5 cases of lobular carcinoma in situ.The postoperative clinical and ultrasonic inspection regularly had no tumors recurrence.Conclusions Ultrasound-guided mammotome system can resect the breast lesions below 3 cm completely and obtain clear pathological diagnosis,with accurate position,and was safe,reliable.
