目的 研究上颌中切牙中等程度缺损时不同颈部情况下,两种桩核材料对牙本质应力的影响.方法 用计算机建立上颌中切牙牙体缺损50%时桩核冠修复,当颈1/3缺损以及颈1/3完整时分别用玻璃纤维桩树脂核、金属桩核,均为烤瓷冠修复共4个模型.模拟临床正常咬(牙合),分析各模型剩余牙本质Von Mises和最大主应力的峰值和分布.结果 4个模型在牙本质表面产生相同的应力分布,两种应力均在舌侧颈缘和根颈1/3出现应力集中区;Von Mises应力从剖面图可见玻璃纤维桩组模型应力分布均匀,而金属桩组模型则明显改变了天然牙的应力分布,但能降低牙本质表面的应力.结论 当颈部牙体保留较多时,建议使用纤维桩;当颈部余留牙体较少、壁较薄时,建议使用金属桩.

Objective To investigate the influence of the stress statement on residual dentin with two different post-core on maxillary central incisor with 50% of tooth defect (gingival place one-third of the crown defect and integrity).Methods Four endodontically treated tooth with 50% of tooth defect were established by the computer:glass fiber post,resin core and porcelain fused to metal crown (PFMC) as restoration or restored by metal post-core and PFMC separately when gingival place one-third of the crown defect and integrity.Clinical load was simulated.Von Mises and the maximum stress were chosen.The stress peak value and stress distribution on residual dentin were investigated with different models.Results Each model showed the same stress distribution on the surface of dentin,the stresses showed significant stress concentration at the lingual cervical margin and at the cervical one-third of the tooth.From the section plane,stress distribution showed homogeneous in the glass fiber restored models,while in metal restored models,stress distribution changed significantly but stress peak was decreasing on the surface of dentine.Conclusions Comparing different post and core materials,fiber post is recommended if enough tooth structure presents at gingival place; and metal post is used if only limited tooth structure presents.
