目的探讨单臂外固定架Ⅰ期治疗多发伤儿童股骨干骨折的适应证及临床疗效.方法回顾性分析26例多发伤并股骨干骨折儿童在C型臂X线机透视下行闭合或切开复位后,采用单臂外固定架固定,术后4～6周部分负重,骨折愈合后拆除外固定架.结果随访结果表明,26例患儿无关节僵直及肌肉无力,无深部感染,无再骨折,无断钉和螺钉拔出,骨折愈合良好,愈合周期为6周～6个月.随访6～24个月,下肢轻度短缩、旋转畸形4例,钉道感染3例,对日常活动无影响.结论单臂外固定架Ⅰ期治疗多发伤儿童股骨干骨折具有创伤小、手术时间短、骨折愈合快、早期功能锻炼、降低并发症、近期疗效肯定等优点.

Objective To investigate the indications and efficacy of single arm outer fix frame on femoral shaft fractures children combined with multiple injuries.Methods The data of twenty-six children with multiple injuries and femoral shaft fractures were retrospectively reviewed.All these patients underwent closed or open reduction with the help of C-arm X-ray machine,and then they were fixed on the single arm fix frame.Partial weight bearing was permitted within 4 to 6 weeks after the operation and the external fixators were removed when the fracture healed.Results All the patients' fractures healed within 6 weeks to 6 months after operation.During the follow up of 6 to 24 months,no complication of ankylosis,deep infection,refracture,breakage of the implant or screw pullout occurred.Mild limb shortening,rotational deformity in 4 cases,3 cases of pin tract infection,and no obvious impact on daily activities.Conclusions Single arm outer fix frame is an effective surgical method for femoral shaft fractures children combined with multiple injuries,with less trauma,shorter operation time,early recovery and few complication.
