目的 探讨大剂量氨溴索在重症肺炎集束化治疗中的应用价值.方法 将45例重症肺炎患者随机分为对照组(n=20)和治疗组(n=25).对照组在给予氧疗、抗感染、营养支持及维持内环境稳定等集束化治疗基础上加用注射用盐酸氨溴索90 mg/d静脉滴注;治疗组在集束化治疗基础上加用注射用盐酸氨溴索90 mg,每6小时1次,静脉滴注.氨溴索治疗疗程为14 d.比较两组患者在治疗前及治疗后第7、14天血C-反应蛋白、降钙素原、氧合指数的变化,并进行统计学处理.结果 两组患者在治疗后第7、14天血C-反应蛋白、降钙素原、氧合指数等方面比较差异均有统计学意义(P<0.05).结论 大剂量氨溴索在重症肺炎集束化治疗中能有效改善氧合,协助抗炎、抗感染作用,可以有效提高重症肺炎的治疗效果.

목적 탐토대제량안추색재중증폐염집속화치료중적응용개치.방법 장45례중증폐염환자수궤분위대조조(n=20)화치료조(n=25).대조조재급여양료、항감염、영양지지급유지내배경은정등집속화치료기출상가용주사용염산안추색90 mg/d정맥적주;치료조재집속화치료기출상가용주사용염산안추색90 mg,매6소시1차,정맥적주.안추색치료료정위14 d.비교량조환자재치료전급치료후제7、14천혈C-반응단백、강개소원、양합지수적변화,병진행통계학처리.결과 량조환자재치료후제7、14천혈C-반응단백、강개소원、양합지수등방면비교차이균유통계학의의(P<0.05).결론 대제량안추색재중증폐염집속화치료중능유효개선양합,협조항염、항감염작용,가이유효제고중증폐염적치료효과.
Objective To investigate the value of large doses of ambroxol on the bundle treatment of severe pneumonia.Methods Forty-five patients with severe pneumonia were randomly divided into control group (n =20) and treatment group (n =25).Patients in control group were given the bundle treatment of oxygen therapy,anti-infection,nutritional support and maintain environmental stability,and intravnous injection of ambroxol 90 mg/d.Patients in treatment group were given the bundle treatment and intravenous injection of ambroxol 90 mg,every 6 hours.The course of ambroxol treatment regimens was 14 d.The blood C-reactive protein,procalcitonin,oxygenation index changes of the two groups,before treatment,on the 7th and 14th day after treatment were compared,and statistically analyzed.Results There were significant differences in the levels of C-reactive protein,procalcitonin,oxygenation index of the two groups on the 7th and 14th day after treatment (P < 0.05).Conclusions Large doses of ambroxol in the bundle treatment of severe pneumonia can effectively improve oxygenation,assist the anti-inflammatory,anti-infective effects,and can effectively improve the therapeutic effect of severe pneumonia.
