目的 探讨NT-proBNP在慢性心力衰竭患者诊断与预后评价中的价值.方法 选择95例慢性心力衰竭患者作为研究对象(心力衰竭),30例非冠心病患者为对照组,对比各组患者的血浆NT-proBNP水平、左室射血分数(LVEF)、左室舒张末内径(LVEDD)；并对慢性心力衰竭患者进行出院后90 d的随访,终点事件是因心力衰竭再次入院或死亡.结果 ①各心功能分级组患者(心功能Ⅱ～Ⅳ级组)入院时血浆NT-porBNP水平[(1350±204) pg/ml,(2932±483) pg/ml、(7522±952) pg/ml]均显著高于对照组[(107±39) pg/ml],左室舒张末内径(LVEDD)[(49.5±5.3)mm、(56.6±6.1) mm、(62.4±8.3) mm]均显著高于对照组[(44.6±3.9)mm],而左室射血分数(LVEF)[(57±7)％、(50±10)％、(45±10)％]均显著低于对照组[(65±5)％],差异有统计学意义(P＜0.05),各心功能分级组之间上述指标的差异也有统计学意义(P＜0.05).②事件组患者入院时血浆NT-porBNP水平[(12257±1906) pg/ml]显著高于非事件组患者[(3618±585) pg/ml],差异有统计学意义(P＜0.05).结论 NT-proBNP的检测对慢性心力衰竭患者的诊断及病情评估起重要作用,NT-proBNP对慢性心力衰竭患者的预后有预测价值.

목적 탐토NT-proBNP재만성심력쇠갈환자진단여예후평개중적개치.방법 선택95례만성심력쇠갈환자작위연구대상(심력쇠갈),30례비관심병환자위대조조,대비각조환자적혈장NT-proBNP수평、좌실사혈분수(LVEF)、좌실서장말내경(LVEDD)；병대만성심력쇠갈환자진행출원후90 d적수방,종점사건시인심력쇠갈재차입원혹사망.결과 ①각심공능분급조환자(심공능Ⅱ～Ⅳ급조)입원시혈장NT-porBNP수평[(1350±204) pg/ml,(2932±483) pg/ml、(7522±952) pg/ml]균현저고우대조조[(107±39) pg/ml],좌실서장말내경(LVEDD)[(49.5±5.3)mm、(56.6±6.1) mm、(62.4±8.3) mm]균현저고우대조조[(44.6±3.9)mm],이좌실사혈분수(LVEF)[(57±7)％、(50±10)％、(45±10)％]균현저저우대조조[(65±5)％],차이유통계학의의(P＜0.05),각심공능분급조지간상술지표적차이야유통계학의의(P＜0.05).②사건조환자입원시혈장NT-porBNP수평[(12257±1906) pg/ml]현저고우비사건조환자[(3618±585) pg/ml],차이유통계학의의(P＜0.05).결론 NT-proBNP적검측대만성심력쇠갈환자적진단급병정평고기중요작용,NT-proBNP대만성심력쇠갈환자적예후유예측개치.
Objective To investigate the diagnostic and prognostic value of N-terminal pro brain natriuretic peptide(NT-proBNP) in patients with chronic heart failure.Methods The plasma levels of NT-proBNP in 95 patients with chronic heart failure and 30 patients who were diagnosed as non coronary heart disease as controls group were detected and the left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter (LVEDD) were measured by cardiac color doppler ultrasound.The occurrence of rehospitalization and death of patients with chronic heart failure was followed-up for 90 days.Results ①The plasma levels of NT-proBNP in patients with chronic heart failure (NYHA Ⅱ-Ⅳ group) who were graded by New York Heart Function Assessment[(1350 ± 204) pg/ml,(2932 ± 483) pg/ml,(7522 ± 952) pg/ml] was significant higher than that of control group [(107 ± 39) pg/ml],LVEDD [(49.5 ± 5.3) mm,(56.6 ± 6.1) mm,(62.4 ± 8.3) mm] was significant higher than that of controls [(44.6 ±3.9)mm],but LVEF[(57.4 ±6.7)％,(50.3 ±9.6)％,(45.2 ± 10.1)％] was significant lower than that of control group [(65.2 ± 4.5) ％].The difference among NYHA Ⅱ group,NYHA Ⅲ group,NYHA Ⅳ group was also statistically significant.②The plasma levels of NT-proBNP in event group were significantly higher than those in non-event group.Conclusions The detection of the plasma levels of NT-proBNP is important for the diagnosis and disease assessment in patients with chronic heart failure,NT-proBNP can provide valuable prognostic information for chronic heart failure.
